摘要
目的:总结体外膜肺氧合(ECMO)病人早期活动的相关证据。方法:计算机检索Up To Date、BMJ Best Practice、国际指南协作网、英国国家卫生与临床优化研究所、苏格兰院际指南网、美国国立指南网、加拿大安大略医学会、美国危重症学会、美国危重病护士协会、Joanna Briggs Institute(JBI)循证卫生保健中心数据库、新西兰指南协作网、the Cochrane Library、PubMed、中国生物医学文献数据库(CBM)、中国知网、中华医学会期刊数据库、维普数据库、体外生命支持组织网站(ELSO)等数据库关于ECMO病人早期活动的指南、专家共识、系统评价、证据总结、随机对照试验、队列研究,经过文献纳入、评价、证据提取及汇总,形成证据总结。结果:共纳入文献11篇,包括指南1篇、专家共识1篇、系统评价3篇、证据总结1篇和5篇原始文献(随机对照研究1篇、队列研究4篇),从运动的必要性、运动的安全性及评估、病人受益性、运动处方、运动管理方案的制定者5个方面形成18条ECMO病人早期活动的最佳证据。结论:临床护理人员在应用ECMO病人早期活动方案过程中应结合病人实际情况进行专业判断,合理参考相关证据,让病人从中获益。
Objective:To summarize the evidence related to early mobilization of extracorporeal membrane oxygenation(ECMO)patients.Methods:The guidelines,expert consensus,systematic reviews,evidence summaries,randomized controlled trials(RCTs),and cohort studies of ECMO patients′early mobilization were retrieved from Up To Date,BMJ Best Practice,International Guidelines Collaboration,UK National Institute for Health and Clinical Excellence,Scottish Intercollegiate Guidelines Network,US National Guidelines Network,Ontario Medical Association,American Society of Critical Care Nurses,American Association of Critical Care Nurses,JBI Evidence-Based Health Care Center Database,New Zealand Guidelines Collaboration Network,the Cochrane Library,PubMed,China Biomedical Literature Database(CBM),China National Knowledge Infrastructure(CNKI),Chinese Medical Association Journal Database,VIP Database,and Extracorporeal Life Support Organization Website(ELSO).Evidence summary was formed by literature inclusion,evaluation,evidence extraction and summary.Results:A total of 11 pieces of literature were included in this study,including 1 guideline,1 expert consensus,3 systematic reviews,1 evidence summary,and 5 original pieces of literature(1 RCT and 4 cohort studies).A total of 18 best pieces of evidence for early mobilization of ECMO patients were formed from 5 aspects,namely sex,exercise safety and evaluation,patient benefit,exercise prescription,and exercise management plan makers.Conclusions:Clinical nurses should make professional judgments in combination with the actual situation of patients in applying the early mobilization program for ECMO patients and make reasonable reference to evidence so that patients can benefit from it.
