摘要
昆布、海藻中含碘量较高,是现代临床运用治疗甲状腺疾病、乳腺类疾病、结节类疾病的常用药对。历代医家运用此药对经验丰富,在不同朝代各有特点,所运用的疾病、剂型也有很大的变化。作者通过整理古籍文献中含有“昆布-海藻”的复方,进行归纳、总结、分析,考究其主治病症及组方历代沿革的变化,并结合现代临床研究探讨了运用富碘药对治疗瘿病的配伍,以期为现代医家诊疗提供些许思路。研究发现,含有“昆布-海藻”药对的复方有171首,有明确方名者81首,其中高频复方有“昆布丸”“破结散”“海藻丸”等。古代医家运用此药对主要用作丸剂、散剂,清代运用汤剂逐渐增多。主治病症以“瘿病”“瘰疬”“疝气”疾病为主。治疗瘿病多配伍海蛤、半夏、松萝、龙胆草、木通、白蔹、连翘等药物,以“昆布丸”“海藻丸”“破结散”等复方运用较多。治疗疝气类疾病,多配伍行气类药物,以“橘核丸”“炼阴丹”运用较多。治疗瘰疬多配伍连翘、玄参、大黄、黄芩,以“昆布散”“木香丸”运用较多。
The iodine content in kelp and seaweed is relatively high,and it is a commonly used drug pair for the treatment of thyroid disease,breast disease and nodular disease in modern clinical practice.Physicians of all dynasties have rich experience in using this medicine,which has its own characteristics in different dynasties,and the diseases and dosage forms used also have great changes.The author summarizes,summarizes,and analyzes the compound prescriptions containing“Kombu-seaweed”in ancient books and documents,studies the main diseases and the changes of the prescriptions in the past dynasties,and combines modern clinical research to discuss the use of iodineenriched medicines in the treatment of galls.Compatibility,in order to provide some ideas for modern doctors diagnosis and treatment.The study found that there were 171 compound prescriptions containing the“Kombu-Seaweed”drug pair,and 81 of them had clear prescription names.Among them,the high-frequency compound prescriptions included“Kombu Pills”,“Pojie San”,“Seaweed Pills”and so on.Ancient doctors used this medicine to be mainly used as pills and powders,and the use of decoctions gradually increased in the Qing Dynasty.The main diseases are“gall disease”,“scrofula”and“hernia”.For the treatment of galls,it is often combined with sea clam,Pinellia,Usnea,Gentiana,Akebia,Radish,Forsythia and other drugs,and compound prescriptions such as“Khumbu Pill”,“Haizao Pill”and“Pojie San”are often used.For the treatment of hernia diseases,it is mostly compatible with Qi promoting drugs,among which“Juhe Pill”and“Lianyin Pill”are mostly used.Forsythia,scrophulariae,rhubarb,and skullcap are often used in the treatment of scrofula,and“Khumbu powder”and“Muxiang pill”are used more.
