摘要
目的:探讨微导管精准立体塑形技术治疗颅内动脉瘤介入栓塞的临床效果。方法:选取2021年3月—2022年6月丰城市人民医院收治的60例颅内动脉瘤患者,按随机数字表法将其分为观察组(30例,行微导管精准立体塑形技术介入栓塞治疗)与对照组(30例,行支架辅助治疗)。对比两组神经功能缺损评分、生活活动能力评分、住院时长,手术前后可溶性细胞间黏附分子-1(sICAM-1)和中枢神经特异性蛋白(S100β)水平,术后Raymond分级、并发症发生率及预后情况。结果:两组术前美国国立卫生研究院卒中量表(NIHSS)评分与日常生活活动能力评定量表(ADL)评分比较,差异均无统计学意义(P>0.05)。两组术后6个月NIHSS评分均低于术前,ADL评分均高于术前,观察组NIHSS评分低于对照组,ADL评分高于对照组,差异均有统计学意义(P<0.05)。观察组住院时长短于对照组(P<0.05)。两组术前血清sICAM-1、S100β水平比较,差异均无统计学意义(P>0.05),两组术后6个月血清sICAM-1、S100β水平均低于术前,且观察组均低于对照组(P<0.05);观察组术后Raymond分级Ⅰ级占比高于对照组(P<0.05);观察组术后并发症发生率为10.00%,低于对照组的36.67%(P<0.05);观察组预后恢复良好率为93.33%,高于对照组的70.00%(P<0.05)。结论:微导管精准立体塑形技术治疗颅内动脉瘤介入栓塞的疗效良好,能改善患者神经功能、日常活动能力,可调节sICAM-1、S100β水平,降低并发症发生率,预后效果好。
Objective:To investigate the clinical effect of microcatheter precision stereoscopic shaping technique in the treatment of intracranial aneurysm interventional embolization.Method:Sixty patients with intracranial aneurysm admitted to Fengcheng People's Hospital from March 2021 to June 2022 were selected and divided into observation group(30 patients,treated with interventional embolization by microcatheter precision stereoscopic shaping technique)and control group(30 patients,treated with stent-assisted treatment)according to the number table method.Neurological deficit score,life activity score,length of hospital stay,levels of soluble intercellular adhesion molecule-1(sICAM-1)and central nervous specific protein(S100β)before and after surgery,Raymond grade after surgery,incidence of complications and prognosis were compared between the two groups.Result:There was no significant difference in preoperative national institutes of health stroke scale(NIHSS)scores and activity of daily living(ADL)scores between the two groups(P>0.05).6 months after surgery,NIHSS score in both groups were lower than before surgery,ADL scores were higher than before surgery,NIHSS score in observation group was lower than control group,ADL score was higher than control group,the differences were statistically significant(P<0.05).The length of hospitalization in the observation group was shorter than that in the control group(P<0.05).There were no significant differences in serum sICAM-1 and S100βlevels between the two groups before surgery(P>0.05),the serum sICAM-1 and S100βlevels in the two groups 6 months after surgery were lower than those before surgery,and the observation group were lower than those in the control group(P<0.05).The proportion of Raymond gradeⅠin observation group was higher than that in control group(P<0.05).The incidence of postoperative complications was 10.00%in the observation group,which was lower than 36.67%in the control group(P<0.05).The good recovery rate of observation group was 93.33%,which was higher than 70.00%of control group(P<0.05).Conclusion:The microcatheter precision stereotaxic technique for interventional embolization of intracranial aneurysms has good efficacy,improves patients'neurological function and daily activity,regulates sICAM-1 and S100βlevels,reduces complication rate,and has good prognosis.
