摘要
目的探讨三维(3D)打印技术在颅内动脉瘤栓塞术的应用。方法选取2019年8月至2020年1月收治于首都医科大学附属宣武医院神经外科出血性脑血管病中心的6名患者,通过采集医学数字成像和通信(DICOM)格式的数据,使用桌面式熔融沉积成型(FDM)3D打印机进行模型打印。根据3D打印模型进行微导管塑形并行动脉瘤栓塞术。由6位专家分别在术前及术中就3D模型及微导管塑形进行打分及评价。术前评价分≥4.5分为优秀、评价分≥3.5分且<4.5分为合格、评价分<3.5分为较差;术中评价单根微导管打分3分及以上为优秀,2分为合格,不足2分为较差。结果 6位专家给予3D模型较高的综合评价[(4.87±0.35)分],认为模型在时效性[(4.67±0.52)分]、真实性[(4.83±0.41)分]、革新性[(5.00±0.00)分]、指导性[(5.00±0.00)分]、实用性[(4.83±0.41)分]方面表现优秀。术中参照3D模型塑形的6例微导管中,仅有1例(眼动脉动脉瘤)因位置欠佳而撤出重新塑形,其余5例在易通过性、位置、稳定性及再塑形情况方面均有良好表现。结论 3D打印模型用于动脉瘤栓塞术前规划有较强的临床意义,可用于术中微导管塑形的指导。
Objective To explore the applied value of three-dimensional(3D)printing technology in preoperative planning of intracranial aneurysm(ICA)embolization.Methods We selected six cases undergoing ICA embolization from August 2019 to January 2020 at the Center for Neurosurgery Hemorrhagic Cerebrovascular Disease of Xuanwu Hospital,Capital Medical University.We collected the data in digital imaging and communications in medicine(DICOM)format then processed model printing by using a desktop fused deposition modeling(FDM)3D printer.Surgeon performed micro-catheter shaping and aneurysm embolization based on 3D printed models.Six experts evaluated and graded the 3D model and the shape of the micro-catheter before and during the operation.The pre-operation rating(the units are in ponits)will be excellent(≥4.5)qualified(≥3.5&<4.5)and poor(<3.5)based on the scores,the during-operation rating(the units are in ponits)will be be excellent(≥3)qualified(2)and poor(<2)based on the scores.Results Experts give high marks to the 3D model in combination with their actual work experience,and give full recognition in aspects such as timeliness(4.67±0.52),authenticity(4.83±0.41),innovative feature(5.00±0.00),instructive(5.00±0.00),and practicality(4.83±0.41).The micro-catheter shaped by the surgeon with reference to the 3D printed model scores highly in terms of accessibility,positioning and stability.Conclusion The 3D printed model has certain clinical significance for the ICA embolization,and it has certain instructive role for the shaping of micro-catheter.It could also be used for clinical teaching of medical students and disease education for patients.
