摘要
目的:分析冠状动脉CT血管造影(Computed Tomography Angiography,CTA)用于心脏血管性疾病的临床准确性。方法:回顾性分析2016年3月~2021年3月本院收治的502例疑似心脏血管性疾病患者。所有患者均行冠状动脉CT血管造影与冠状动脉造影(金标准),对比冠状动脉CT血管造影与金标准的诊断结果差异,分析冠状动脉CT血管造影对于心脏血管性疾病、管腔狭窄的诊断价值。结果:冠状动脉造影结果中为阳性患者478例,阴性24例;冠状动脉CT血管造影中阳性469例,阴性33例;冠状动脉CT血管造影诊断的准确率为93.43%、灵敏度为97.28%、特异度为83.33%、阳性预测值为99.15%及阴性预测值为60.61%;在CTA检查成功的502例患者中,有24例患者为管壁不狭窄、极小狭窄15例、中度狭窄278例、重度狭窄123例、管腔闭塞者62例,管腔狭窄者共478例,通过与冠状动脉造影检查结果对比,发现CTA对血管中度以上狭窄分支诊断的符合率为96.86%(463/478)。结论:冠状动脉CT血管造影对心脏血管性疾病的诊断具有较高的准确性、灵敏度和特异度。
Objective: To analyze the clinical accuracy of coronary CT Angiography for cardiovascular diseases. Methods: 502 patients with suspected cardiovascular disease admitted to our hospital from March 2016 to March 2021 were retrospectively analyzed. All patients underwent coronary CT angiography and coronary angiography(gold standard). The difference in diagnostic results between coronary CT angiography and gold standard was compared to analyze the diagnostic value of coronary CT angiography for cardiovascular diseases. Results: There were 478 positive patients and 24 negative patients in coronary angiography. Coronary CT angiography was positive in 469 cases and negative in 33 cases. The accuracy, sensitivity, specificity, positive predictive value and negative predictive value of coronary CT angiography were 93.43%, 97.28%, 83.33%, 99.15% and 60.61% respectively;Among 502 patients with successful CTA examination, there were 24 patients with non-stenosis and 15 patients with minimal stenosis. 278 patients with moderate stenosis;123 cases with severe stenosis;62 cases had lumen occlusion;A total of 478 patients with lumen stenosis were found to have a coincidence rate of 96.86%(463/478) in the diagnosis of moderate or higher stenosis branches by CTA compared with the results of coronary angiography. Conclusion: Coronary CT angiography has high accuracy, sensitivity and specificity in the diagnosis of cardiovascular diseases.
