摘要
目的探究个体化饮食干预对糖尿病肾病维持性血液透析患者人体成分指标、营养状况及炎性因子的影响。方法选取郑州大学第一附属医院肾脏内科2019年5月至2021年5月间105例进行维持性血液透析的糖尿病肾病患者作为研究对象,采用随机数字表法分为2组。对照组52例给予维持性血液透析常规护理干预,观察组53例在对照组基础上增加个体化饮食干预。2组均干预6个月后,比较2组患者营养状况、人体成分指标和炎症因子水平。结果观察组干预后转铁蛋白(TRF)、白蛋白(ALB)、前白蛋白(PA)、每日蛋白分解率(nPCR)水平均高于对照组(P<0.05);观察组干预后肱三头肌皮褶厚度(TSF)、中臂围(MAC)、中臂肌围(MAMC)水平均高于对照组(P<0.001);观察组干预后血清肿瘤坏死因子-α(TNF-α)、超敏C-反应蛋白(hs-CRP)、白细胞介素-6(IL-6)水平均低于对照组(P<0.001)。结论个体化饮食干预对糖尿病肾病维持性血液透析患者效果确切,能够有效改善患者营养状况和人体成分指标,降低炎性因子水平。
Objective To study the effects of individualized dietary intervention on body composition indexes,nutritional status and inflammatory factors in maintenance hemodialysis patients with diabetic nephropathy.Methods A total of 105 patients with diabetic nephropathy who received maintenance hemodialysis from May 2019 to May 2021 in nephrology department were randomly given routine nursing alone(control group,52 patients)or in combination with individualized dietary intervention(observation group,523 patients).The nutritional status,inflammatory factor levels and body composition indexes were compared between the two groups after 6 months of intervention.Results Compared with the control group,the individualized dietary intervention increased transferrin,albumin and prealbumin levels,normalized protein catabolic rate,triceps skinfold thickness,middle arm circumference and middle arm muscle circumference,and decreased serum tumor necrosis factor-α,high-sensitivity C-reactive protein and interleukin-6 levels in the observation group(P<0.05 or P<0.001).Conclusion Individualized dietary intervention can effectively improve the nutritional status and body composition indexes and reduce the levels of inflammatory factors in maintenance hemodialysis patients with diabetic nephropathy.
