摘要
目的观察柴芍六君子汤治疗慢性萎缩性胃炎的临床效果及对证候积分的影响。方法选择2019年4月—2020年2月在湖南中医药大学第二附属医院治疗的慢性萎缩性胃炎患者106例,依据随机数字表法分为观察组和对照组,每组53例。对照组给予常规西药治疗,观察组给予柴芍六君子汤治疗,2组患者均连续服用4周。比较2组患者临床疗效、症状消失时间,治疗前后证候积分、炎性因子、胃动力指标及不良反应。结果治疗4周后,观察组患者总有效率为96.23%,高于对照组的77.36%(χ^(2)=8.230,P=0.004);观察组患者症状消失时间为(5.21±0.64)d,短于对照组的(10.49±0.98)d(t=32.844,P<0.001)。治疗4周后,2组患者胃脘痞满、胃胀、嗳气泛酸、反酸、食欲减退积分均低于治疗前,且观察组低于对照组(P<0.01);2组患者白介素-6(IL-6)、白介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)水平均低于治疗前,且观察组低于对照组(P<0.01);2组患者胃蛋白酶原-Ⅰ(PG-Ⅰ)水平均高于治疗前,胃蛋白酶原-Ⅱ(PG-Ⅱ)水平均低于治疗前,且观察组升高/降低幅度大于对照组(P<0.01)。观察组患者不良反应总发生率为3.77%,低于对照组的16.98%(χ^(2)=4.970,P=0.026)。结论柴芍六君子汤治疗慢性萎缩性胃炎的效果较好,可有效改善证候积分及炎性因子水平,提高胃功能,且药物安全性较高,值得推广应用。
Objective To observe the clinical effect of Chaishao Liujunzi decoction in the treatment of chronic atrophic gastritis and its influence on symptom scores.Methods A total of 106 patients with chronic atrophic gastritis who were treated in the Second Affiliated Hospital of Hunan University of Chinese Medicine from April 2019 to February 2020 were selected,according to the random number table method,they were divided into observation group and control group,with 53 cases in each group.The control group was given conventional western medicine treatment,while the observation group was given Chaishao Liujunzi decoction,and both groups were given continuous treatment for 4 weeks.The clinical efficacy,symptom disappearance time,symptom scores,inflammatory factors,gastric motility indexes before and after treatment and adverse reactions were compared between the two groups.Results After 4 weeks of treatment,the total effective rate in the observation group was 96.23%,which was higher than 77.36%in the control group(χ^(2)=8.230,P=0.004);The symptom disappearance time of the observation group was(5.21±0.64)days,which was shorter than(10.49±0.98)days of the control group(t=32.844,P<0.001).After 4 weeks of treatment,the scores of epigastric fullness,bloating,belching pantothenic acid,acid regurgitation and anorexia in both groups were lower than those before treatment,and the observation group were lower than the control group(P<0.01);The interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)levels of two groups were lower than those before treatment,and the observation group were lower than the control group(P<0.01);The levels of pepsinogen-Ⅰ(PG-Ⅰ)in the two groups were higher than those before treatment,and the levels of pepsinogen-Ⅱ(PG-Ⅱ)were lower than those before treatment,and the increase/decrease in the observation group were greater than those in the control group(P<0.01).The total incidence of adverse reactions in the observation group was 3.77%,which was lower than 16.98%in the control group(χ^(2)=4.970,P=0.026).Conclusion Chaishao Liujunzi decoction has a good effect in the treatment of chronic atrophic gastritis,can effectively improve the symptom score and the level of inflammatory factors,improve gastric function,and has high drug safety,which is worthy of popularization and application.
