摘要
中医认为脾为“气血生化之源”,能量代谢与脾主运化的功能密切相关。在动脉粥样硬化(AS)病变过程中,巨噬细胞糖代谢以瓦博格效应为主,即发生糖代谢重编程,而糖代谢重编程被视为“脾虚”的生物学基础。巨噬细胞糖代谢重编程驱动炎症反应是AS的核心事件,炎症反应导致的白细胞介素(IL)-1β释放可与胆固醇竞争获得从巨噬细胞输出的三磷酸腺苷结合盒转运体A1(ABCA1),抑制胆固醇逆转运,促进泡沫细胞形成,导致AS斑块的形成,该病变过程又与“脾为生痰之源”的病变过程相契合。文章旨在聚焦糖代谢重编程驱动巨噬细胞炎症反应,探讨“脾虚生痰”所致AS病变过程中,“脾虚”与“痰浊”之间的机制联系。
Traditional Chinese medicine believes that ‘the spleen is the source of qi and blood’, and energy metabolism is closely related to the function of the spleen in governing transportation and transformation. During the pathological process of atherosclerosis(AS), the sugar metabolism of macrophages is dominated by the Warburg effect, that is, macrophage glycobolism reprogramming. Glycobolism reprogramming is the biological basis of ‘spleen deficiency’. Macrophage glycometabolism reprogramming to drive inflammation is the core event of AS. The release of IL-1β caused by the inflammatory response can compete with cholesterol for the ABCA1 transporter exported from macrophages, inhibit cholesterol reverse transport, promote foam cell formation, and lead to the formation of atherosclerotic plaques. This pathological process is consistent with the pathological process of ‘the spleen is the source of phlegm production’. This paper aims to focus on glycobolism reprogramming to drive the inflammatory response of macrophages, and to explore the mechanism link between ‘spleen deficiency’ and ‘phlegm syndrome’, in the process of atherosclerosis caused by ‘spleen deficiency generating phlegm’.
