摘要
急性冠脉综合征(Acute Coronary Syndrome,ACS)是冠心病急症。李军教授临证概括为“虚、痰、瘀(郁)、热”致病,虚证以气虚、阳虚、阴虚为主,实证为痰浊、瘀血、气郁、热蕴,以脏腑阴阳虚损、气血两伤为本,痰瘀郁热、邪气痼结为标,形成本虚标实、虚实夹杂证候。急性期、亚急性期、慢性期病机特点分别为“虚瘀痰热”“虚瘀痰滞”“虚瘀饮”。胸阳不振、心脉痹阻为病机关键。临证明辨病因病机,重视气血辨证。治疗强调扶阳活血、益气养阴、辛开苦降、凉血活血,临床疗效显著。
Acute coronary syndrome(ACS)is an emergency of coronary heart disease.In the clinic,Professor Li Jun summarizes that the syndrome is caused by“deficiency,phlegm,blood stasis(stagnation of evil qi),and heat evil”.The deficiency syndrome mainly consists of qi deficiency,yang deficiency,and yin deficiency,and the excess syndrome includes phlegm turbidity,blood stasis,qi stagnation,and heat evil.The deficiency of yin and yang in zang-fu organs and the injury of qi and blood are the origin,while the phlegm stasis and heat evil and the disturbance of evil qi are the superficiality,forming the syndrome of asthenia in origin and sthenia in superficiality and deficiency-excess in complexity.The pathogenesis characteristics of acute phase,subacute phase,and chronic phase of ACS are“deficiency,blood stasis phlegm,and heat evil”,“deficiency,blood stasis,and phlegm stagnation”,and“deficiency,blood stasis,and fluid retention”,respectively.Lack of yang in chest and heart vessel obstruction are the key to pathogenesis.In the clinic,the syndrome differentiation of qi and blood should be focused on to differentiate the pathogenesis.The treatment emphasizes supporting yang and activating blood circulation,tonifying qi and yin,pungent dispersing and bitter descending,and cooling blood.The treatment has a significant effect in the clinic.
