摘要
目的:观察健脾愈萎汤联合铋剂四联疗法对慢性萎缩性胃炎(CAG)患者Hp根除、胃蛋白酶原及临床症状的影响。方法:选取经胃镜以及病理检查确诊为CAG,经^(14)C呼气试验证实为Hp阳性、中医辨证分型属脾胃虚弱证的80例患者为研究对象,按随机数字表法分为治疗组和对照组,每组各40例。治疗组以健脾愈萎汤联合铋剂四联治疗,对照组以四联疗法治疗,疗程均为10 d,治疗后评估Hp根除情况、胃蛋白酶原的变化情况及临床症状改善情况。结果:治疗组Hp根除率为95.0%,对照组为75.0%,治疗组明显优于对照组(P<0.05);与治疗前相比,两组均能有效提高血清胃蛋白酶原Ⅰ水平、胃蛋白酶原Ⅰ和胃蛋白酶原Ⅱ的比值(PGR),降低胃蛋白酶原Ⅱ水平(P<0.05),但治疗组在降低胃蛋白酶原Ⅱ水平、提高PGR方面明显优于对照组(P<0.05)。两组均能有效改善患者的胃脘疼痛症状(P<0.05),但治疗组能明显改善胃脘痞闷、食欲减退、大便稀溏、乏力、气短懒言症状(P<0.05),对照组改善不明显(P>0.05)。结论:健脾愈萎汤联合铋剂四联疗法能有效提高CAG患者的Hp根除率,改善血清中胃蛋白酶原的表达水平,缓解患者的临床症状,提高临床疗效。
Objective:To observe the effects of Jianpi Yuwei Decoction combined with bismuth quadruple therapy on Helicobacter pylori(Hp),pepsinogen(PG)and clinical symptoms in patients diagnosed chronic atrophic gastritis(CAG).Methods:Eighty patients diagnosed with CAG by gastroscope and pathological examination,confirmed Hp positive by ^(14)C breath test,and the Traditional Chinese Medicine syndrome type was spleen-stomach weakness syndrome.They were divided into the experimental group and the control group according to the random number table method.with 40 cases in each group.The experimental group was treated with Jianpi Yuwei Decoction combined with bismuth quadruple therapy,and the control group was treated with quadruple therapy.The course of treatment was 10 days.After treatment,the eradication of Hp,the change of PG and the improvement of clinical symptoms were evaluated.Results:The eradication rate of Hp in the experimental group was 95.0%and that in the control group was 75.0%.The experimental group was significantly better than the control group(P<0.05).Compared with before treatment,both groups could effectively increase the levels of serum PGⅠand PGⅠ/PGⅡ(PGR),reduce the level of PGⅡ(P<0.05),but the experimental group was significantly better than the control group in reducing the content of PGⅡand increasing the level of PGR(P<0.05).Both groups could effectively improve the symptoms of stomach pain(P<0.05),but the experimental group could significantly improve the symptoms of Stomach stuffiness,loss of appetite,loose stools,hypodynamia,shortness of breath and lazy word(P<0.05),and the control group improved not obvious(P>0.05).Conclusion:Jianpi Yuwei Decoction combined with bismuth quadruple therapy can effectively improve the eradication rate of Hp in patients with atrophic gastritis,improve the expression level of PG in serum,improve the clinical symptoms of patients,and the clinical efficacy.
