摘要
目的:观察化浊解毒方治疗慢性糜烂性胃炎(Chronic erosive gastritis,CEG)浊毒内蕴证患者的临床疗效及上皮细胞间质化(Epithelial-Mesenchymal-transition,EMT)相关蛋白:钙黏附蛋白E(E-cadherin,E-cad)、Snail1、Twist的表达水平,探讨化浊解毒方治疗慢性糜烂性胃炎浊毒内蕴证的作用机制,为浊毒理论的临床应用提供理论依据。方法:将68例CEG浊毒内蕴证患者随机分为治疗组和对照组各34例,治疗组给予口服化浊解毒方,对照组口服阿拉坦五味丸,3个月为一个疗程,两组均连续服用2个疗程。观察两组治疗前后症状积分、中医证候疗效、胃镜及病理疗效及胃粘膜EMT相关蛋白E-cad、Snail1、Twist表达水平变化。结果:治疗组症状积分、中医证候疗效、胃镜疗效、病理疗效均优于对照组。与对照组比较,治疗组胃粘膜E-cad蛋白表达水平明显上调。胃粘膜Snail1蛋白、Twist蛋白表达明显下调。结论:化浊解毒方能明显改善CEG浊毒内蕴证患者的临床症状、胃镜及病理情况,其作用机制可能与上调E-cad蛋白,下调Snail1、Twist蛋白,阻止上皮细胞向间质细胞转化有关。
Objective:To observe the clinical efficacy of Huazhuo Jiedu foumula on chronic erosive gastritis(CEG)patients with"Zhuoduneiyun"syndrome and the expressions of Epithelial-Mesenchymal-Transition(EMT)related proteins,E-cadherin、Snail1 and Twist protein,to explore the mechanism of Huazhuo Jiedu foumula on CEG,and provide clinical basis.Methods:68 CEG patients with"Zhuoduneiyun"syndrome were randomly divided into the treatment group and the control group,34 cases in each group.The patients in the treatment group were orally given Huazhuo Jiedu foumula,those in the the control group were orally given Alatan Wuwei pills for two courses,3 months in one course.After treatment,the symptoms scores before and after the treatment,the efficacy of TCM syndromes,gastroscopic and pathological effects,and the expression levels of E-cad,Snail1 and Twist in gastric mucosa were observed.Results:Compared with the control group,the symptom score,TCM syndrome efficacy,gastroscopic efficacy and pathological efficacy were better in the treatment group,the expression level of gastric mucosal E-cad protein in the treatment group was significantly up regulated,the expression levels of Snail1 protein and Twist protein in gastric mucosa in the treatment group were significantly down regulated.Conclusion:Huazhuo Jiedu foumula can significantly improve the clinical symptoms,endoscopy and pathology of CEG patients with"Zhuoduneiyun"syndrome.The mechanism may be related to up-regulating E-cad protein,down-regulating Snail1 and Twist proteins,and preventing epithelial cells from transforming into stromal cells.
