川崎病患儿不同程度冠脉病变的相关因素分析及预测模型构建 被引量:6

Related factors analysis and prediction model construction of coronary artery disease in children with Kawasaki disease

摘要 目的 明确川崎病患儿冠脉病变的危险因素,分析不同程度病变与各项危险因素之间的相关性,并构建川崎病患儿不同程度冠脉病变的风险预测模型。方法 选取2016年6月—2020年6月在湖州市中心医院就诊的175例川崎病的患儿,根据是否有冠脉病变进行分组,分为对照组和观察组。根据降钙素原、D-二聚体、Pro-BNP、年龄、性别、川崎病类型、静注人免疫球蛋白(IVIG)的起始时间、使用糖皮质激素、发热持续时间、C-反应蛋白(CRP)、血红蛋白、血沉、白细胞计数、血清白蛋白及血小板进行单因素和多因素logistic回归分析,根据logistic回归分析模型,采用R软件构建川崎病患儿不同程度冠脉病变风险的列线图模型。结果 单因素分析结果显示,降钙素原、D-二聚体、Pro-BNP、年龄、性别、发热持续时间、CRP、血红蛋白、血沉及血小板是川崎病患儿冠脉病变的影响因素,其中降钙素原、D-二聚体、Pro-BNP、发热持续时间、血红蛋白及血小板是其中主要的影响因素(P<0.05)。冠脉病变程度与D-二聚体、血沉、Pro-BNP及降钙素原呈正相关关系(P<0.05)。结论 川崎病患儿并发冠脉病变主要的高危因素为降钙素原、D-二聚体、Pro-BNP、发热持续时间、血红蛋白及血小板,临床上需要结合高危因素做预测因子。 Objective To identify the risk factors of coronary artery disease in children with Kawasaki disease,analyze the correlations between different degrees of lesions and various risk factors,and construct the risk prediction model of different degrees of coronary artery disease in children with Kawasaki disease.Methods A total of 175 children with Kawasaki disease treated in Huzhou Central Hospital from June 2016 to June 2020 were selected and divided into control group and observation group according to complicating with coronary artery disease or not.According to procalcitonin,D-dimer,Pro-BNP,age,gender,type of Kawasaki disease,start time of intravenous immunoglobulin(IVIG),use of glucocorticoid,duration of fever,C-reactive protein(CRP),hemoglobin,erythrocyte sedimentation rate,white blood cell count,serum albumin,and platelet,univariate analysis and multivariate logistic regression analysis were conducted.According to logistic regression analysis model,nomographic model for the risk of different degrees of coronary artery disease in children with Kawasaki disease were constructed by R software.Results Univariate analysis showed that procalcitonin,D-dimer,Pro-BNP,age,gender,duration of fever,CRP,hemoglobin,erythrocyte sedimentation rate,and platelet were influencing factors of coronary artery disease in children with Kawasaki disease,among which procalcitonin,D-dimer,Pro-BNP,duration of fever,hemoglobin,and platelet were the main influencing factors(P<0.05).The degree of coronary artery lesions was positively correlated with D-dimer,erythrocyte sedimentation rate,Pro-BNP,and procalcitonin(P<0.05).Conclusion The main risk factors of coronary artery disease in children with Kawasaki disease include procalcitonin,D-dimer,Pro-BNP,duration of fever,hemoglobin,and platelet,which can be used as predictive factors.
