摘要
目的探讨白内障患者超声乳化联合人工晶状体植入术(IOL)术后并发干眼症的影响因素。方法抽取2018年1月至2018年12月在新乡医学院第三附属医院实施超声乳化联合IOL术后并发干眼症的50例白内障患者纳入发生组, 另抽取同期实施超声乳化联合IOL术后未并发干眼症的50例白内障患者纳入未发生组。回顾性分析两组患者的临床资料, 经单因素及多因素分析研究白内障患者超声乳化联合IOL术后并发干眼症的影响因素。结果发生组糖尿病、睑板腺功能障碍、术前C反应蛋白(CRP)水平、手术切口位置、结膜松弛分级与未发生组比较, 差异有统计学意义(P<0.05);经Logistic回归分析结果显示, 糖尿病、睑板腺功能障碍、术前CRP水平高、透明角膜切口、3级结膜松弛是白内障患者超声乳化联合IOL术后并发干眼症的影响因素(OR>1, P<0.05)。经临床干预、治疗1个月后, 发生组治愈23例(46.00%), 显效18例(36.00%), 有效9例(18.00%)。结论糖尿病、睑板腺功能障碍、术前CRP水平高、透明角膜切口、3级结膜松弛是白内障患者超声乳化联合IOL术后并发干眼症的影响因素, 且术后并发干眼症患者经临床治疗后症状均可改善。
Objective To investigate the influencing factors of xerophthalmia after phacoemulsification combined with intraocular lens implantation(IOL)for cataract.Methods Fifty cataract patients with xerophthalmia after phacoemulsification combined with IOL treated in the Third Affiliated Hospital of Xinxiang Medical University from January 2018 to December 2018 were selected as occurrence group.Another 50 patients without xerophthalmia after phacoemulsification combined with IOL in the same period were included in the non occurrence group.The clinical data of patients in the two groups were analyzed retrospectively,and the influencing factors of xerophthalmia after phacoemulsification combined with IOL implantation in cataract patients were analyzed by univariate and multivariate analysis.Results There were significant differences in diabetes mellitus,meibomian gland dysfunction,preoperative C-reactive protein(CRP)level,incision location and conjunctival relaxation grade between the occurrence group and the non occurrence group(P<0.05).Results of Logistic regression analysis showed that diabetes mellitus,meibomian gland dysfunction,high preoperative CRP level,clear corneal incision and conjunctival laxity 3 stage were the influencing factors of xerophthalmia after phacoemulsification combined with IOL implantation in cataract patients(OR>1,P<0.05).After 1 month of clinical intervention and treatment,23 cases(46.00%)were cured,18 cases(36.00%)got marked therapeutic effects,and 9 cases(18.00%)got therapeutic effects in the occurrence group.Conclusions Diabetes mellitus,meibomian gland dysfunction,high preoperative CRP level,clear corneal incision,and conjunctival laxity 3 stage are the influencing factors of xerophthalmia in cataract patients after phacoemulsification combined with IOL implantation,and postoperative symptoms of xerophthalmia patients can be alleviated by clinical treatment.
