摘要
目的真武汤加地龙治疗肺癌并发恶性胸腔积液的疗效及对血清IL-18、TNF-α和肠道菌群的影响。方法选取2019年6月-2020年6月收治的60例肺癌并发恶性胸腔积液患者临床资料进行分析,对照组(30例)给予静滴长春瑞滨和顺铂抗癌与对症治疗,研究组(30例)在对照组基础上配合真武汤加地龙治疗,比较2组疗效、中医证候积分、不良反应、生存质量和血清IL-18、TNF-α水平,16S rRAN测序检测肠道菌群多样性。结果研究组治疗总有效率为63.33%,较对照组的40%显著增高(P<0.05);2组治疗后中医证候积分均低于治疗前,且以研究组指标下降更显著(P<0.05);研究组与对照组不良反应总发生率无显著差异(P>0.05);研究组认知功能等生存质量各项评分均高于对照组(P<0.05);2组治疗后血清IL-18和TNF-α水平均降低,且以研究组下降更显著(P<0.05);与对照组比较,研究组菌群多样性更丰富。结论真武汤加地龙治疗肺癌并发恶性胸腔积液可显著改善临床症状,降低血清IL-18和TNF-α水平,改善菌群多样性,提升临床治疗效果。
Objective To explore the effect of Zhenwu Decoction plus earthworm in the treatment of lung cancer complicated with malignant pleural effusion and its influence on the levels of serum IL-18,TNF-α and intestinal flora.Methods The clinical data of 60 patients with lung cancer and malignant pleural effusion who were treated from June 2019 to June 2020 were selected for analysis.The control group (30 cases) was given the intravenous infusion of vinorelbine and cisplatin for the anticancer and symptomatic treatment,while the study group (30 cases) were treated with Zhenwu Decoction plus earthworm on the basis of the control group.The curative effect,TCM syndrome scores,adverse reactions,scores of quality of life and levels of serum IL-18 and TNF-α were compared between the two groups,and the diversity of intestinal flora was detected by 16S rRAN sequencing.Results The total effective rate in the study group was 63.33%,which was significantly higher than that in the control group (P<0.05).The TCM syndrome scores of both groups after treatment were lower than before treatment,and the indicators in the study group were decreased more significantly (P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).The scores of quality of life such as cognitive function in the study group were higher than those in the control group (P<0.05).After treatment,the levels of serum IL-18 and TNF-α in the two groups were decreased,and the decrease was more significant in the study group (P<0.05).Compared with the control group,the diversity of intestinal flora in the study group was more abundant.Conclusion In the treatment of lung cancer complicated with malignant pleural effusion,Zhenwu Decoction plus earthworm can significantly improve patients’ clinical symptoms,reduce the levels of serum IL-18 and TNF-α,improve the diversity of intestinal flora as well as the clinical treatment effect.
