摘要
目的观察应用行动导向教学法进行健康指导在老年经皮冠脉介入治疗(PCI)患者管理中的应用效果。方法选取2019年9月—2021年9月在六安市人民医院进行PCI的老年冠心病患者78例,采用数字表法随机分成观察组与对照组,每组39例。对照组采取常规健康指导,观察组应用行动导向教学法进行健康指导。比较2组干预前后的健康行为量表(压力管理、健康责任感、人际关系、心理健康、营养与躯体活动)评分、西雅图心绞痛问卷(SAQ)(活动能力受限、疾病认知程度、心绞痛稳定、心绞痛发作与满意度)评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分及术后主要不良心血管事件发生情况。结果干预前,2组健康行为、SAQ、SAS和SDS评分差异均无统计学意义(P>0.05)。干预后,2组患者健康行为的6个维度评分、SAQ量表的5个维度评分均显著高于同组干预前(P<0.05),观察组健康行为的6个维度评分和SAQ量表的5个维度评分均显著高于对照组(P>0.05);2组患者SAS和SDS评分明显低于同组干预前(P>0.05),观察组SAS和SDS评分均显著低于对照组(P<0.05);观察组术后主要不良心血管事件发生率显著低于对照组(P<0.05)。结论应用行动导向教学法进行健康指导在老年患者行PCI介入术管理中的临床效果较好,可改善患者健康行为和生活质量,缓解患者焦虑和抑郁负性情绪,降低术后主要心血管不良事件的发生率。
Objective To observe the application effects of action-oriented teaching method for health guidance in the management of elderly patients with percutaneous coronary intervention(PCI).Methods 78elderly patients with coronary heart disease who underwent PCI in People’s Hospital of Lu’an City from September2019to September2021were selected and randomly divided into observation group and control group by the digital table method,with39cases in each group.The control group received routine health guidance,while the observation group received health guidance with actionoriented teaching method.The scores of the Health Behavior Scale(stress management,health responsibility,interpersonal relationship,mental health,nutrition and physical activity),Seattle Angina Questionnaire(SAQ)(limited activity,disease cognition,angina stability,Angina pectoris attack and satisfaction),Self-Rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)before and after intervention,and the incidence of major adverse cardiovascular events after operation were compared between the two groups.Results Before the intervention,there was no significant difference in the scores of the health behavior,SAQ,SAS and SDS between the two groups(P>0.05);After the intervention,the scores of six dimensions of health behavior and five dimensions of SAQ of the two groups were significantly higher than those of the same group before the intervention(P<0.05),and the scores of SAS and SDS of the two groups were significantly lower than those of the same group before the intervention(P<0.05).The scores of six dimensions of health behavior and five dimensions of SAQ of the observation group were significantly higher than those of the control group(P<0.05),and the scores of SAS and SDS were significantly lower than those of the control group(P<0.05).The incidence of major adverse cardiovascular events in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The application of action-oriented teaching method for health guidance in the management of PCI in elderly patients has a good clinical effect,which can improve patients’health behavior and quality of life,alleviate their negative emotions of anxiety and depression,and reduce the incidence of major postoperative cardiovascular adverse events.
