摘要
目的探讨脑小血管病MRI总负荷与认知障碍的相关性。方法2019-01—2021-07对沧州市中心医院200例脑小血管病患者(脑小血管病组)和100例健康居民(脑组织正常组)进行研究。比较2组患者基本资料(年龄、高血压史、脑梗死史、高脂血史、TC指标含量、HDL指标),简易智力状态检查(MMSE)和蒙特利尔认知评估量表(MoCA)评分及不同磁共振总负荷分数患者MMSE和MoCA评分。结果与脑组织正常组比较,脑小血管病组患者年龄大,TC指标含量、HDL指标含量低,高血压史、脑梗死史、高脂血史构成比较高,差异均有统计学意义(P<0.05)。脑小血管病组患者MMSE视空间及执行功能得分、注意与计算得分、语言得分、延迟回忆得分、定向得分和MoCA定向能力得分、记忆能力得分、注意力和计算力得分、回忆能力得分、语言能力得分显著低于脑组织正常组,差异均有统计学意义(P<0.05)。不同总负荷分数患者MMSE和MoCA评分差异有统计学意义(P<0.05)。结论脑小血管病MRI总负荷与认知障碍有明显的相关性。
Objective To explore the correlation between total MRI load of cerebral small vessel disease and cognitive impairment,anxiety and depression.Methods From January 2019 to July 2021,200 patients with cerebrovascular disease(cerebrovascular disease group)and 100 healthy residents(normal brain tissue group)in Cangzhou Central Hospital were studied.The basic data(age,history of hypertension,history of cerebral infarc tion,history of hyperlipidemia,content of TC index and HDL index),the MMSE and MoCA scores,the MMSE and MoCA scores of patients with different total magnetic resonance load scores were compared.Results Compared with the normal brain tissue group,the patients in the small vascular disease group were older,the content of TC index and HDL index were lower,and the history of hypertension,cerebral infarction and hyperlipidemia were higher,with statistically significant differences(P<0.05).Small brain vascular disease patients’MMSE space and executive function score,attention and calculation points,language score,delayed recall score,score and MoCA orientation ability score,memory ability score,attention and computing power,memory score ability score,language ability score were significantly lower than normal brain tissue group,the differences were statistically significant(P<0.05),MMSE and MoCA scores had statistical significance in patients with different total load scores(P<0.05).Conclusion The total MRI load of cerebral small vessel disease is correlated with cognitive impairment,anxiety and depression.
