摘要
目的:分析放大内镜联合窄带成像(ME-NBI)检查在胃早期肿瘤性病变诊断中的价值。方法:选取深圳市宝安区松岗人民医院2015年1月-2020年1月收治的可疑胃早期肿瘤性病变并取得活检病理结果的患者共50例,均依次接受白光内镜和ME-NBI检查,以活检病理结果为金标准,对比白光内镜和ME-NBI对胃早期肿瘤性病变的诊断效能。结果:以活检病理结果为金标准,50例患者中共检查出早期胃癌45例,良性胃病5例;ME-NBI检查早期胃癌的敏感度、特异度、阳性预测值及阴性预测值均高于白光内镜检查(95.56%vs77.78%,80.00%vs60.00%,97.73%vs94.59%,66.67%vs23.08%),且与金标准检查结果一致性高(Kappa=0.777,P<0.05);不同胃小凹Sakaki分型下,ME-NBI检查对早期胃癌的检出率高于白光内镜检查(P<0.05)。结论:与白光内镜检查相比,放大内镜联合窄带成像检查可提高胃早期肿瘤性病变诊断价值,提高sakaki分型D型胃小凹的检出率,值得进一步推广。
Objective:To analyze the value of magnifying endoscopy combined with narrow band imaging(ME-NBI)in the diagnosis of early gastric tumors.Method:A total of 50 patients with suspected early gastric neoplastic lesions,who admitted to Songgang People’s Hospital,Bao’an District,Shenzhen from January 2015 to January 2020 and obtained biopsy pathological results were selected.All patients received white light endoscopy and ME-NBI in order,and the biopsy pathological results were the gold standard.the diagnostic efficacy of white light endoscopy and ME-NBI for early gastric tumors were compared.Result:Taking the biopsy pathological results as the gold standard,a total of 45 patients with early gastric cancer,5 patients with benign gastric disease.The sensitivity,specificity,positive predictive value and negative predictive value of ME-NBI in the detection of early gastric cancer were higher than those of white light endoscopy(95.56%vs 77.78%,80.00%vs 60.00%,97.73%vs 94.59%,66.67%vs 23.08%),and they were highly consistent with the results of the gold standard test(Kappa value=0.777,P<0.05).Different stomachs of the small concave Sakaki classification,the detection rate of early gastric cancer by ME-NBI examination was higher than that of white light endoscopy(P<0.05).Conclusion:Compared with white light endoscopy,magnifying endoscopy combined with narrow-band imaging can improve the diagnostic value of early gastric tumors and the detection rate of stomachs of the small concave sakaki classification with D type,is worthy of further promotion.
