摘要
目的:探讨补肾活血法联合穴位贴敷治疗不明原因复发性流产的效果。方法:选取2017年1月-2018年9月仙桃市中医医院收治的76例不明原因复发性流产患者,按照随机数字表法分配为对照组和治疗组,每组38例。对照组给予地屈孕酮片,治疗组患者在对照组基础上加用补肾活血法联合穴位贴敷进行内外合治。比较两组保胎成功率、主要症状的缓解时间、内分泌激素水平[β-人绒毛膜促性腺激素(β-hCG)、孕酮(P)]、子宫血流动力学指标[子宫动脉收缩期峰值流速(PSV)、子宫动脉的阻力系数(RI)、动脉搏动指数(PI)],监测免疫调节相关指标[CD4^(+)T辅助细胞Th17(Th17/CD4^(+))、CD4^(+)调节性T细胞Treg(Treg/CD4^(+))、辅助性T细胞17/调节性T细胞(Th17/Treg)]水平。结果:两组保胎成功率比较,差异无统计学意义(P>0.05)。治疗组腹部疼痛缓解时间、出血停止时间、腰酸坠胀缓解时间均短于对照组,差异均有统计学意义(P<0.05)。治疗后,两组β-hCG、P水平均高于治疗前,且治疗组均高于对照组,差异均有统计学意义(P<0.05)。治疗后,治疗组RI、PI均低于对照组,PSV高于对照组,差异均有统计学意义(P<0.05)。治疗后,治疗组Th17/CD4^(+)、Th17/Treg均低于对照组,Treg/CD4^(+)高于对照组,差异均有统计学意义(P<0.05)。结论:补肾活血法联合穴位贴敷治疗不明原因复发性流产疗效确切,利于缓解症状,初步推断其通过调节内分泌功能、改善子宫胎盘灌注状态、调节机体的免疫平衡以产生保胎效果,且保胎成功率高。
Objective:To discuss the effect of kidney-tonifying and blood-activating method combined with acupoint application in the treatment of unexplained recurrent abortion.Method:A total of 76 patients with unexplained recurrent abortion admitted to Xiantao Hospital of Traditional Chinese Medicine from January 2017 to September 2018 were selected,they were divided into control group and treatment group according to random number table method,with 38 patients in each group.The control group was given Dydrogesterone Tablets,and the treatment group was given kidney-tonifying and blood-activating method combined with acupoint application for internal and external combined treatment.The success rate of fetal preservation,relief time of main symptoms,endocrine hormone levels[β-human chorionic gonadotropin(β-hCG),progesterone(P)],uterine hemodynamic indexes[peak systolic velocity(PSV),resistance index(RI),pulsatility(PI)]were compared between two groups;the levels of CD4^(+)T helper cell Th17(Th17/CD4^(+)),CD4^(+)regulatory T cell Treg(Treg/CD4^(+)),and helper T cell 17/regulatory T cell(Th17/Treg)were compared between two groups.Result:There was no significant difference in the success rate of fetal preservation between two groups(P>0.05).The relief time of abdominal pain,bleeding stop time and lumbar acid swelling relief time of the treatment group were shorter than those of the control group,the differences were statistically significant(P<0.05).After treatment,the levels of β-hCG and P of both groups were higher than those before treatment,those of the treatment group were higher than those of the control group,the differences were statistically significant(P<0.05).After treatment,the RI and PI of the treatment group were lower than those of the control group,and the PSV of the treatment group was higher than that of the control group,the differences were statistically significant(P<0.05).After treatment,the Th17/CD4^(+)and Th17/Treg of the treatment group were lower than those of the control group,while the Treg/CD4^(+)of the treatment group was higher than that of the control group,the differences were statistically significant(P<0.05).Conclusion:The method of kidney-tonifying and blood-activating method combined with acupoint application for the treatment of unexplained recurrent abortion has a definite effect,which is conducive to the relief of symptoms.It is preliminarily inferred that it produces the effect of fetal preservation by regulating the endocrine function,improving the condition of uterine placenta perfusion and regulating the immune balance of the body,and the success rate of fetal preservation is high.
