摘要
目的观察孟鲁司特钠联合布地奈德对哮喘患儿肺功能、炎性因子及免疫功能的影响。方法回顾性选择2018年8月—2020年10月于大冶市人民医院儿科住院治疗的小儿哮喘患儿90例,根据药物治疗方案不同分为观察组和对照组,每组45例。对照组采用布地奈德混悬液雾化吸入治疗,观察组在对照组基础上加用孟鲁司特钠口服治疗,2组患儿疗程均为2周。采用肺功能仪检测2组患儿治疗前后第1秒用力呼气容积(FEV 1)、用力肺活量(FVC)及呼气流量峰值(PEF)等肺功能指标;采用酶联免疫吸附法检测2组患儿治疗前后白介素(IL)-4、IL-8、IL-13及白三烯B4(LTB4)等炎性因子表达水平;采用流式细胞分析仪检测2组患儿治疗前后T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))表达水平。结果治疗2周后,2组患儿的FEV 1、FVC、PEF、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均升高,IL-4、IL-8、IL-13、LTB4及CD8^(+)水平均降低,且观察组升高/降低幅度大于对照组,差异均有统计学意义(P<0.01)。结论孟鲁司特钠联合布地奈德治疗可明显改善哮喘患儿肺功能及免疫功能,减轻炎性反应。
Objective To observe the effects of montelukast sodium combined with budesonide on lung function,inflammatory factors and immune function in children with asthma.Methods 90 children with asthma who were hospitalized in the Department of Pediatrics,People's Hospital of Daye from August 2018 to October 2020 were retrospectively selected,and they were divided into observation group and control group according to different drug treatment plans,with 45 cases in each group.The control group was given budesonide suspension aerosol inhalation therapy,and the observation group was given montelukast sodium oral therapy on the basis of the control group,the course of treatment for both groups was 2 weeks.Lung function indexes such as FEV 1,FVC and PEF were detected by spirometer in the two groups before and after treatment,and enzyme-linked immunosorbent assay was used to detect IL-4,IL-8,IL-13 and LTB4 and other inflammatory factors,and flow cytometry was used to detect the expression levels of T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))in the two groups before and after treatment.Results After 2 weeks of treatment,the FEV 1,FVC,PEF,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)of the two groups of children all increased,The levels of IL-4,IL-8,IL-13,LTB4 and CD8^(+)were decreased,and the increase/decrease in the observation group was greater than that in the control group,and the differences were statistically significant(P<0.01).Conclusion Montelukast sodium combined with budesonide treatment can significantly improve lung function and immune function in children with asthma,and reduce inflammatory response.
