摘要
目的观察小青龙汤联合督脉灸治疗痰湿内蕴型肺癌咳嗽的临床疗效及对疼痛、炎症反应、生活质量的影响。方法将2020年3月—2021年6月在绵阳市中医医院呼吸科治疗的90例痰湿内蕴型肺癌咳嗽患者随机分为对照组30例、观察A组30例、观察B组30例。对照组给予复方磷酸可待因口服溶液治疗,观察A组在对照组治疗基础上加用小青龙汤治疗,观察B组在观察A组治疗基础上加用督脉灸治疗,3组疗程均为2周。观察3组患者治疗前后咳嗽严重程度评分、咳嗽视觉模拟评分(VAS评分)、疼痛数字评分(NRS评分)、炎性相关指标[白细胞(WBC)、中性粒细胞百分比(NEUT%)、中性粒细胞绝对值(NEUT)、降钙素原(PCT)、C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、生活质量评分(LCQ评分)变化,比较3组患者咳嗽治疗效果,记录3组患者不良反应发生情况。结果治疗后,3组患者咳嗽严重程度评分、咳嗽VAS评分、NRS评分、LCQ评分和WBC、NEUT%、NEUT、CRP、PCT、IL-6、TNF-α水平均明显低于治疗前(P均<0.05),观察B组上述指标均明显低于其他2组(P均<0.05),观察A组均明显低于对照组(P均<0.05)。对照组、观察A组、观察B组总有效率分别为63.4%(19/30)、80.0%(24/30)、90.0%(27/30),观察B组治疗总有效率明显高于其他2组(P均<0.05),观察A组明显高于对照组(P<0.05)。结论小青龙汤联合督脉灸可更有效地缓解痰湿内蕴型肺癌患者咳嗽症状,减轻患者疼痛及炎症反应,可明显改善患者生活质量。
Objective It is to observe the clinical efficacy of Xiaoqinglong Decoction combined with governor meridian moxibustion in the treatment of lung cancer cough of type of phlegm-dampness internal accumulation and its effects on pain,inflammatory response and quality of life.Methods A total of 90 patients with lung cancer cough type of phlegm-dampness internal accumulation treated in the Department of Respiratory Medicine of Mianyang Hospital of Traditional Chinese Medicine from March 2020 to June 2021 were randomly divided into a control group of 30 cases,an observation group A of 30 cases,and an observation group B of 30 cases.The control group was treated with compound codeine phosphate oral solution,the observation group A was treated with Xiaoqinglong Decoction on the basis of the treatment of the control group,and the observation group B was treated with governor meridian moxibustion on the basis of treatment of the observation group A.all the 3 groups were treated for 2 weeks.The changes in the cough severity score,cough visual analog score(VAS score),pain digital score(NRS score),inflammatory related indicators[white blood cells(WBC),neutrophil percentage(NEUT%),neutrophil absolute count(NEUT),procalcitonin(PCT),C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],quality of life score(LCQ score)before and after treatment were observed in the 3 groups,the cough treatment effect of the 3 groups were compared,and the occurrence of adverse reactions in the 3 groups were recorded.Results After treatment,the cough severity scores,cough VAS scores,NRS scores,LCQ scores and the levels of WBC,NEUT%,NEUT,CRP,PCT,IL-6 and TNF-αin the 3 groups were significantly lower than those before treatment(all P<0.05),the above indexes in the observation group B were significantly lower than those in the other two groups(all P<0.05),and observation group A was significantly lower than that in the control group(P<0.05).The total effective rates of the control group,observation group A,and observation group B were 63.3%(19/30),80.0%(24/30),and 90.0%(27/30),respectively.The total effective rate of observation group B was significantly higher than that of the other two groups(P<0.05),the observation group A was significantly higher than the control group(P<0.05).Conclusion Xiaoqinglong Decoction combined with moxibustion on the governor meridian can more effectively relieve cough symptoms,relieve pain and inflammatory response,and can significantly improve the quality of life of patients with lung cancer of type of phlegm-dampness internal accumulation.
