摘要
目的:探析《金匮要略》治疗妇人腹痛的遣方用药规律,总结张仲景治疗妇人腹痛的学术思想,为深入学习经方、运用经方、提高临床治疗妇人腹痛疗效提供借鉴思路。方法:将《金匮要略》妇人病三篇中治疗妇人腹痛的方剂录入Microsoft Excel 2019建立数据库,对单味药物进行频次、功效、四气五味、脏腑归经的统计。使用Clementine 12.0、IBM SPSS Statistics 25软件对高频中药进行关联分析和聚类分析,多角度探析治疗妇人腹痛的用药规律与特征。结果:共纳入处方13首,涉及中药30味,单味药中芍药、当归、桂枝使用频率最高,药味以甘、苦、辛为主,药物归经主归于脾经、肝经和心经,药物功效主要为补血、补气、活血等。药物关联分析显示常用药对为:芍药-桂枝、川芎-当归-芍药。聚类分析得到三组聚类方,其药性符合仲景学术思想。结论:本研究从多角度探析《金匮要略》治妇人腹痛的用药规律,总结张仲景治疗妇人腹痛的学术思想,即根据气血失和、肝脾不调的总病机,制定调和气血、肝脾同治的总治则,同时不忘顾护胃气。其治法以补法和消法为主,融补消于一炉;其遣方多立足于气血,重在调理气和血的关系;其用药寒温并行,温药通经散寒,寒药通利散结;其剂型灵活多变,不拘常法。同时,挖掘符合仲景学术思想的新组方,为妇人腹痛病的临床用药提供重要参考价值。
Objective:To explore and analyze the prescriptions in the treatment of abdominal pain in women in Jinkui Yaolue(金匮要略),to provide a reference for the in-depth researches and the application of the prescriptions and improvement of clinical efficacy.Methods:The prescriptions treating abdominal pain in women from three articles of Jinkui Yaolue(金匮要略)were entered into Microsoft Excel 2019 to establish a database,then the using frequency,the pharmacological effects,four types of Qi and five flavors,and the meridian tropism of single-use drugs were analyzed.Clementine 12.0 and IBM SPSS Statistics 25 software were used for the correlation analysis and the clustering analysis on high-frequency drugs,the medication rules and characteristics of drugs for abdominal pain in women were studied.Results:13 prescriptions and 30 TCM drugs were selected.Drugs with the highest using frequency were Chinese herbaceous peony,angelica sinensis and cassia twig.The main characteristics of the highest frequency drugs were Gan,Ku and Xin flavor.The meridian tropisms of the highest frequency drugs were spleen,liver and heart,while the main effects were Buxue,Buqi and Huoxuehuayu.The correlation analysis results showed that the frequently used drug pairs were Chinese herbaceous peony-Assia twig,and Ligusticum wallichii-Angelica sinensis-Chinese herbaceous peony.In the cluster analysis,three new prescriptions were obtained.Conclusion:In this paper,the medication rules treating the abdominal pain in women were studied,Zhongjing theory was summarized:according to the pathogenesis of Qixueshihe and Ganpishitiao,the therapy principle of Tiaoheqixue and Ganpitongzhi was selected,and Guhuweiqi was also taken into consideration.The main treatments for the abdominal pain in women were Bu and Xiao,and their combination.The main medication rules were to regulate Qi and Xue,and to use Han drugs combined with Wen drugs.The Wen drugs had Tongjingsanhan effect,while the Han drugs had Tonglisanjie effect.The prescriptions were modified according to the clinical situation.This study explored the new prescriptions treating the abdominal pain in women,which provides an important reference for the clinical use.
