基于迭代重建算法对胸部CT行低剂量扫描图像质量及辐射剂量的影响 被引量:4

Effect of Iterative Reconstruction Algorithm on Image Quality and Radiation Dosage of Low-dose Scan for Chest CT

摘要 目的:探讨基于迭代重建算法对胸部CT行低剂量扫描图像质量及辐射剂量的影响。方法:选取60例行胸部CT平扫的患者作为研究对象,随机分为三组,每组20例。A组为常规剂量组(120 kVp,300 mAs),采用滤波反投影算法(FBP)重建;B组和C组均为低剂量组(80 kV,300 mA),分别采用FBP重建和迭代重建算法(SAFIRE)重建。对比分析三组图像肺窗及纵隔窗的图像客观质量[噪声(SD)、信噪比(SNR)及对比噪声比(CNR)]和主观质量差异,并比较常规剂量组和低剂量组有效辐射剂量(ED)差异,对2名医生评价的一致性行Kappa检验。结果:低剂量组ED较常规剂量组降低约82.35%,差异有统计学意义(P<0.05);C组SD较B组降低45.62%,差异有统计学意义(P<0.05),SNR和CNR较B组分别增加39.57%和28.64%,差异均有统计学意义(P<0.05),而与A组比较SD、SNR及CNR相差不大,差异均无统计学意义(P>0.05),且2名医生对图像质量主观评价的一致性较好(肺窗Kappa=0.862,纵隔窗Kappa=0.841)。结论:应用SAFIRE算法对胸部CT行低剂量扫描,可以在保证图像质量的前提下,显著降低辐射剂量,具有较高的临床应用价值。 Objective To explore the effect of iterative reconstruction algorithm on image quality and radiation dose of low dose scan of chest CT.Method Sixty patients with chest CT plain scan were randomly divided into A、B、C three groups,20 cases in each group.A group was a conventional dose group(120 kVp,300mAs),which was reconstructed by filtering backprojection algorithm(FBP),B group and C group were both low dose groups(80 kVp,300mAs),respectively,using FBP reconstruction and iterative reconstruction algorithm(SAFIRE)reconstruction.The objective quality[noise(SD),signal-to-noise ratio(SNR)and contrast-noise ratio(CNR)]and subjective quality differences of the images of lung window and mediastinal window in the three groups were compared.the difference of effective radiation dose(ED)in the conventional dose group and low dose group was compared.a consistent Kappa test was performed for 2 doctors.Results The ED of low dose group was about 82.3%lower than that of conventional group(P<0.05),and that of C group was 45.6%lower than that of B group(P<0.05),SNR and CNR increased by 39.5%and 28.6%,respectively,All differences were statistically significant(P<0.05),The SD、SNR and CNR were not different from those of A group,All differences were not statistically significant(P>0.05),And two doctors had better agreement on subjective evaluation of image quality(lung window Kappa=0.86,mediastinal window Kappa=0.84).Conclusion Using SAFIRE algorithm to scan chest CT at low dose can significantly reduce radiation dose on the premise of ensuring image quality,and has high clinical application value.
