Effects of Dengzhan Shengmai Capsule(灯盏生脉胶囊) Combined with Salkubatroxartan Sodium Tablet on Serum Levels of APN,MMP-9 and BNP in Patients with Coronary Heart Disease Complicated with Chronic Heart Failure

摘要目的探讨灯盏生脉胶囊结合沙库巴曲缬沙坦钠片对冠心病合并慢性心衰患者血清脂联素(APN)、基质金属蛋白酶-9(MMP-9)和脑钠肽(BNP)水平的影响。方法以2019年1月—2020年12月医院收治的冠心病合并慢性心力衰竭患者196例为研究对象,依据随机对照法将患者分为治疗组和对照组,每组98例。对照组采用西药常规治疗,积极控制血压等危险因素,在利尿、强心、扩血管等药物治疗的基础上,口服沙库巴曲缬沙坦钠治疗。沙库巴曲缬沙坦钠片100 mg, 2次/d。治疗组在对照组治疗的基础上,口服灯盏生脉胶囊治疗,0.36 g, 3次/d。两组均按疗程治疗2周。比较两组治疗前后气血血瘀证证候积分比较;比较两组治疗前后心功能水平,包括左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)水平;比较治疗前后APN、MMP-9和BNP水平;比较治疗前后生活质量评价量表(SF-36)评分,比较两组治疗疗效。结果治疗组治疗总有效率高于对照组(P<0.05),两组治疗后胸痛、胸闷、气短、心悸不安、神疲力乏、面色淡白和口唇紫黯证候积分较治疗前降低,且治疗组降低幅度优于对照组(P<0.05);两组治疗后心功能指标LVESD、LVEDD较治疗前降低,LVEF较治疗前升高。且治疗组改善幅度优于对照组(P<0.05);两组治疗后血清APN较治疗前升高,MMP-9和BNP水平较治疗前降低。且治疗组改善幅度优于对照组(P<0.05);两组治疗后社会功能、生理功能、日常活动、社会活动、心理健康和活力评分较治疗前升高。且治疗组升高幅度优于对照组(P<0.05)。结论灯盏生脉胶囊结合沙库巴曲缬沙坦钠可最大幅度的改善患者的临床症状,控制病情,改善心脏功能,提高血清APN水平,降低MMP-9和BNP水平,提高患者的生活质量和治疗疗效,值得借鉴。 Objective To investigate the effects of Dengzhan Shengmai Capsule(灯盏生脉胶囊)combined with Salkubatroxartan Sodium Tablet on serum levels of APN,MMP-9 and BNP in patients with coronary heart disease complicated with chronic heart failure.Methods A total of 196 patients with coronary heart disease complicated with chronic heart failure admitted to our hospital from January 2019 to December 2020 were selected as the research objects.According to the randomized control method,the patients were divided into treatment group and control group,with 98 cases in each group.The control group was treated with conventional western medicine,and risk factors such as blood pressure were actively controlled.On the basis of diuretic,cardiotonic,vasodilator and other drug treatments,the patients were treated with Salkubatroxartan Sodium Tablet orally(100 mg,twice a day).The treatment group was additionally treated with Dengzhan Shengmai Capsule(0.36 g,three times a day).Both groups were treated for 2 weeks.The scores of syndrome of Qi,blood and blood stasis were compared between the two groups before and after treatment.Left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD)and left ventricular end-systolic diameter(LVESD)were compared between the two groups before and after treatment.Serum levels of adiponectin(APN),matrix metalloproteinase-9(MMP-9)and brain natriuretic peptide(BNP)were compared before and after treatment.The quality of life scale(SF-36)scores before and after treatment were compared,and the therapeutic efficacy of the two groups was compared.Results The total effective rate of the treatment group was higher than that of the control group(P<0.05).The scores of chest pain,chest tightness,shortness of breath,palpitation,tiredness,pale complexion and purple lip dark syndrome in the two groups after treatment were lower than those before treatment,and the reduction rate of the treatment group was better than that of the control group(P<0.05).After treatment,cardiac function indexes LVESD and LVEDD were decreased and LVEF was increased in the two groups.The improvement of the treatment group was better than that of the control group(P<0.05).After treatment,serum levels of APN and MMP-9 and BNP in both groups were higher than those before treatment.The improvement of the treatment group was better than that of the control group(P<0.05).The scores of social function,physiological function,daily activity,social activity,mental health and vitality in both groups were higher after treatment than those before treatment.The increase rate in treatment group was higher than that in the control group(P<0.05).Conclusion Dengzhan Shengmai Capsule combined with Salkubatroxartan Sodium Tablet can greatly improve the clinical symptoms of patients,control the disease,improve cardiac function,increase serum APN level,reduce levels of MMP-9 and BNP,improve the quality of life and therapeutic efficacy of patients,which is worthy of reference.
