高通量血液透析联合左卡尼汀治疗维持性血液透析患者贫血的疗效观察 被引量:4

摘要 目的探讨高通量血液透析与左卡尼汀联合治疗维持性血液透析患者贫血的效果。方法选取本院2018年12月至2019年12月收治的64例维持性血液透析伴贫血患者,并依据不同透析器与是否使用左卡尼汀分为4组,各16例。甲组仅给予低通量透析,乙组给予低通量透析联合左卡尼汀,丙组给予高通量血液透析,丁组给予高通量血液透析联合左卡尼汀。比较4组血常规指标(红细胞计数、红细胞容积及血红蛋白)水平和血清生化指标(血清肌酐、尿素氮、超敏C反应蛋白及β;微球蛋白)水平。结果治疗后,甲组贫血无明显改善;与甲组比较,乙组、丙组、丁组血常规指标中的红细胞计数、红细胞容积及血红蛋白指标均有改善,差异有统计学意义(P<0.05);丁组血清肌酐、尿素氮、超敏C反应蛋白及β;微球蛋白四项血清生化指标均优于乙组、丙组,差异有统计学意义(P<0.05)。结论与低通量血液透析方法比较,高通量血液透析与左卡尼汀联合对维持性血液透析患者贫血症状有理想改善作用,可有效优化血常规指标,纠正患者的贫血情况,值得临床推广应用。 Objective To explore the therapeutic effect of high flux hemodialysis combined with levocarnitine on anemia in maintenance hemodialysis patients. Methods 64 maintenance hemodialysis patients with anemia admitted in our hospital from December 2018 to December 2019 were selected and they were divided into four groups according to different dialyzers and whether to use L-carnitine, with 16 cases in each group. Group A patients were only given low-throughput dialysis, group B patients were given low-throughput dialysis combined with levocarnitine, group C patients were given high-throughput hemodialysis, and group D patients were given high-throughput hemodialysis liquid dialysis combined with levocarnitine. The blood routine indexes(RBC count, RBC volume and hemoglobin) and serum biochemical indexes(serum creatinine, urea nitrogen, hypersensitive C-reactive protein and β;-microglobulin) were compared among the four groups. Results There was no significant improvement in anemia after treatment in group A;Compared with group A, the RBC count, red blood cell volume and hemoglobin in Group B, group C and group D were significantly improved(P<0.05);four serum biochemical indexes of serum creatinine, urea nitrogen, hypersensitive C-reactive protein and β;-microglobulin in group D were better than those in group B and group C, the difference was statistically significant(P<0.05). Conclusion Compared with the method of low flux hemodialysis, the combination of high flux hemodialysis and levocarnitine can improve the anemia symptoms of maintenance hemodialysis patients. It can effectively optimize the blood routine indexes and correct the anemia of patients. It is worthy of clinical application.
