摘要
目的探究左卡尼汀联合人重组促红细胞生成素在血液透析肾性贫血患者中的应用效果。方法选取2019年4月-2021年8月于我院行血液透析的肾性贫血患者40例,采用随机数字表法将其分为研究组(n=20)及对照组(n=20)。对照组于血液透析后皮下注射人重组促红细胞生成素,研究组在其基础上加用左卡尼汀。对比两组临床疗效、心功能指标[左室舒张末期内径(LVEDD)、左心房内经(LAD)、左室射血分数(LVEF)、左室心肌质量指数(LVMI)]、贫血相关指标及不良反应。结果研究组治疗总有效率为90.00%,高于对照组的60.00%,有统计学差异(P<0.05);研究组治疗后LAD、LVEDD、LVMI水平均低于对照组,LVEF高于对照组,血红蛋白含量及红细胞压积均大于对照组,有统计学差异(P<0.05);两组不良反应比较,无统计学差异(P>0.05)。结论联合治疗在血液透析肾性贫血治疗中获得较好效果,患者心功能各指标均得到有效改善,有助于促进心肌恢复,恢复心功能,纠正贫血症状,不良反应少,安全性高,值得推广应用。
Objective:To explore the effect of l-carnitine combined with human recombinant erythropoietin in hemodialysis patients with renal anemia.Methods:A total of 40 patients with renal anemia who underwent hemodialysis in our hospital from April 2019 to August 2021 were included and divided into a study group(n=20)and a control group(n=20)by random number table method.The control group was subcutaneously injected with human recombinant erythropoietin after hemodialysis,and the study group was additionally given l-carnitine.The clinical efficacy,cardiac function indexes[left ventricular end-diastolic diameter(LVEDD),left atrial meridian(LAD),left ventricular ejection fraction(LVEF),left ventricular myocardial mass index(LVMI)],anemia-related indexes and adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the study group was 90.00%,which was higher than 60.00%in the control group,and the difference was statistically significant(P<0.05).The hemoglobin content and hematocrit in the control group were higher than those in the control group,and the difference was statistically significant(P<0.05);there was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion:L-carnitine combined with human recombinant erythropoietin can achieve good effect in the treatment of hemodialysis patients with renal anemia,and all indicators of cardiac function can be effectively improved,which is helpful to promote myocardial recovery,restore cardiac function,and correct anemia symptoms.It has few adverse reactions and high safety,and is worthy of popularization and application.
