摘要
目的风湿性心脏病患者在临床瓣膜置换术治疗过程中实施单极射频消融术治疗对永久性心房纤颤的治疗意义。方法该次研究对象从该院2014年6月—2021年6月治疗的风湿性心脏病患者中方便选择530例患者,采用随机分配法分为两组,每组265例,联合组利用心脏病瓣膜置换术、单极射频消融术治疗,普通组单纯实施常规心脏病瓣膜置换术治疗,比较两组患者治疗前后患者心功能指标、临床指标及并发症发生率。结果经治疗后,两组心功能指标明显优于治疗前,且联合组优于普通组,差异有统计学意义(P<0.05);两组患者临床指标对比中出血量、引流量、手术时间、体外循环时间差异无统计学意义(P>0.05),但联合组住院时间(17.65±1.54)d短于普通组,差异有统计学意义(t=55.884,P<0.05);联合组并发症发生率为1.51%低于普通组26.04%,差异有统计学意义(χ^(2)=67.122,P<0.05)。结论在临床风湿性心脏病患者实施瓣膜置换术基础上结合单极射频消融术治疗,能实现患者术后永久性心房纤颤治疗,使患者心功能尽快康复。
Objective The significance of monpolar radiofrequency ablation in patients with rheumatic heart disease for permanent atrial fibrillation.Methods The study subjects were selected conveniently from the hospital's treatment of rheumatic heart disease patients from June 2014 to June 2021.A total of 530 patients were enrolled,and they were divided into two groups by random allocation,with 265 cases in each group,the combined group was treated with heart valve replacement and unipolar radiofrequency ablation,and the normal group was treated with conventional heart valve replacement.The cardiac function indexes,clinical indexes and complication rate of the two groups of patients before and after treatment were compared.Results After treatment,the heart function indexes of the two groups were significantly better than those before treatment,and the combined group was better than the normal group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the amount of bleeding,drainage,operation time,and extracorporeal circulation time between the two groups of patients in clinical indicators(P>0.05).The length of stay in the combined group was(17.65±1.54)d shorter than that in the normal group,and the difference was statistically significant(t=55.884,P<0.05).The complication rate of the combined group was 1.51%lower than that of the normal group 26.04%,the difference was statistically significant(χ^(2)=67.122,P<0.05).Conclusion Based on the implementation of valve replacement surgery in clinical patients with rheumatic heart disease,combined with unipolar radiofrequency ablation treatment,it can achieve permanent atrial fibrillation treatment after surgery and make the patient's heart function recover as soon as possible.
