摘要
探讨了社会医学教学知识存在的困境,总结了两大问题一是在公共卫生业务工作中职业用途欠突出,包括公卫人员对社会医学知识的掌握不够且缺乏相应专门对口的职业条线、缺乏充分研究;二是教学核心主题,即社会因素对健康的影响,在教学知识体系中核心地位不显著;部分周边教学内容被其他课程共享。主要原因是公共卫生子学科按健康问题分化而将社会医学所研究的具体健康问题划分到了其他课程名下,同样公共卫生业务条线也主要按健康问题划分。此外,医学与社会科学之间的学科壁垒使得社会医学对社会科学知识的借鉴不够充分。这两者导致在核心主题上缺乏持续的研究和知识总结。最终建议社会医学积极参与学科分化,要加强教学内容中核心主题的建设,多引入社会学、医学人类学等社会科学理论。
This article analyzed problems in course knowledge of social medicine and found that the usage of social medicine knowledge in public health work was not widely recognized nor linked specifically to any established profession,and the core theme,i.e.effect and mechanism of social determinants on health,was not dominant in course knowledge.There are two main causes of these predicaments.First,public health has evolved and differentiated into various sub-disciplines by substantive health issues,consequently social-related health problems traditionally identified as social medicine knowledge are now categorized as their own sub-disciplines.Professions are organized also by substantive health issues.Second,disciplinary division between medicine and social science has hindered social medicine to absorb and apply social science knowledge in exploring and reviewing core theme research in depth.Finally,it is suggested that social medicine should take part in disciplinary evolution and be a more distinctive sub-discipline of public health,strengthen core theme research,and introduce more social science knowledge from sociology and medical anthropology.
