摘要
目的分析大黄芒硝散外敷联合常规抗感染治疗在剖宫产术后腹壁切口感染、裂开患者中的临床效果。方法选取2018年1月1日至2020年12月31日南京医科大学附属苏州医院(苏州市立医院)收治的87例剖宫产术后腹壁切口感染、裂开患者作为研究对象进行回顾性分析,按其治疗方式分为对照组(43例)和观察组(44例)。对照组接受抗感染、常规换药治疗;观察组接受抗感染、常规换药治疗外,另加大黄芒硝散外敷切口。比较两组患者的切口愈合等级、细菌培养转阴时间、硬结消失时间、切口完全愈合时间、住院时间、炎症因子指标[C反应蛋白(CRP)、降钙素原(PCT)、白细胞计数(WBC)]、再次清创缝合率等。结果观察组患者的切口愈合情况优于对照组,差异有统计学意义(P<0.05)。观察组的细菌培养转阴时间、硬结消失时间、切口完全愈合时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。治疗1周后,观察组的CRP、PCT、WBC水平低于对照组,差异有统计学意义(P<0.05);治疗2周后,两组的CRP、PCT、WBC水平比较,差异无统计学意义(P>0.05)。观察组的再次清创缝合率低于对照组,差异有统计学意义(P<0.05)。结论大黄芒硝散外敷联合常规抗感染治疗能有效促进剖宫产术后感染、裂开切口的愈合,降低再次清创缝合率,缩短住院时间。
Objective To analyze the clinical effect of Rhubarb-Mirabilite Powder combined with conventional anti-infective treatment in patients with abdominal wall incision infection or dehiscence after cesarean section.Methods A total of 87 patients with abdominal wall incision infection or dehiscence after cesarean section treated in the Affiliated Suzhou Hospital of Nanjing Medical University(Suzhou Municipal Hospital)from January 1,2018 to December 31,2020 were selected for retrospective analysis.According to their treatment methods,they were divided into the control group(43 cases)and the observation group(44 cases).The control group received anti-infection and conventional dressing change.The observation group received anti-infection and conventional dressing change treatment,and external application of Rhubarb-Mirabilite Powder was given to the incisions additionally.The incision healing grade,bacterial culture negative time,induration disappearance time,incision complete healing time,hospitalization time,inflammatory factor indexes(C-reactive protein[CRP],procalcitonin[PCT],white blood cell count[WBC]),the re-debridement and suture rate were compared between the two groups.Results The incision healing grade of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The bacterial culture negative time,induration disappearance time,incision complete healing time,hospitalization time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After one week of treatment,the levels of CRP,PCT and WBC in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 2 weeks of treatment,there was no significant difference in the levels of CRP,PCT and WBC between the two groups(P>0.05).The rate of redebridement and suture in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The external application of Rhubarb-Mirabilite Powder combined with conventional anti-infective treatment can effectively promote the healing of infected incisions after cesarean section,reduce the rate of re-debridement and suture,and shorten the hospitalization time.
