摘要
目的:观察腰椎间盘突出症患者病变肢体红外熵变规律及与疼痛、功能评分间的相关性。方法:选取符合诊断及纳入标准的腰椎间盘突出症患者与健康受试者,采集受试者肢体红外热像图,对图像进行增强、降噪与多尺度熵分析,与患者肢体疼痛、功能评分进行相关性分析。结果:腰椎间盘突出症患者患侧肢体小腿后外侧区、足底区、足背区、大腿后侧区多尺度熵值较患者健侧肢体及健康受试者相关区域多尺度熵值升高,差异有统计学意义(P<0.05)。患侧肢体小腿前外侧区多尺度熵值较患者健侧肢体及健康受试者相关区域多尺度熵值,差异无统计学意义(P>0.05)。患者健侧肢体相关感性兴趣区较健康受试者多尺度熵值间差异无统计学意义(P>0.05)。腰椎间盘突出症患者患侧肢体大腿后侧、小腿后外侧、足底区多尺度熵值与ODI、VAS评分正相关,与JOA评分负相关。结论:红外热像多尺度熵分析法可深度提取病变肢体的信息,对腰椎间盘突出症患者病情的诊断与评估具有指导价值,为进一步认识与研究疾病提供了新思路。
Objective:To explore the change of infrared entropy of limbs in patients with lumbar disc herniation and its correlation with pain and function scores.Methods:Patients with lumbar disc herniation and healthy subjects who met the criteria for diagnosis,inclusion and exclusion were selected.Infrared thermography of the limbs of the subjects was collected,and the images were analyzed for enhancement,noise reduction and multi-scale entropy,then the correlation analysis was conducted with the patients’limb pain and functional scores.Results:The multi-scale entropy values of the posterolateral calf,plantar,dorsal foot and posterior thigh of the affected limbs of patients with lumbar disc herniation were higher than those of the healthy limbs and related regions of healthy subjects,with statistical difference(P<0.05).There was no significant difference in the multi-scale entropy of the anterolateral calf of the affected limb compared with the healthy limb of the patient and the related regions of the healthy subjects(P>0.05).There was no statistical difference between the multi-scale entropy values of the healthy limbs area of patients and those of the healthy subjects(P>0.05).The multi-scale entropy values of the posterior thigh,posterolateral calf and plantar of the limbs affected by lumbar disc herniation was positively correlated with ODI and VAS scores,and negatively correlated with JOA score.Conclusion:Infrared thermography multi-scale entropy analysis method can extract the information of diseased limbs in depth,which has a guiding value for the diagnosis and evaluation of lumbar disc herniation,and provides a new idea for further unders-tanding and research of the disease.
