摘要
【目的】探讨256iCT薄层扫描联合三维重建技术对孤立性肺结节(SPN)的诊断价值。【方法】选取2016年12月-2021年6月本院门诊及住院患者中CT表现为SPN的患者167例作为研究对象,均经病理证实,并分为良性组(60例)和恶性组(107例)。收集患者年龄等一般资料,256iCT薄层扫描检测患者结节类型、结节大小、分叶征、毛刺征、瘤肺界面、血管集束征等。Kappa分析256iCT薄层扫描联合三维重建技术对SPN良恶性判定与病理检查结果的一致性。分析256iCT薄层扫描联合三维重建技术对SPN良恶性的诊断价值。【结果】与良性组比较,恶性组年龄、结节类型、结节大小、分叶征、毛刺征、瘤肺界面、血管集束征等因素差异有统计学意义(P<0.05)。256iCT薄层扫描诊断结果与病理诊断呈较高一致性(Kappa=0.685,P<0.05),联合检测诊断结果与病理诊断呈高度一致性(Kappa=0.802,P<0.05)。256iCT薄层扫描联合三维重建技术敏感度较256iCT薄层扫描单独诊断升高(P<0.05)。【结论】256iCT薄层扫描联合三维重建技术是一种无创性检测SPN性质的技术,对判断SPN良恶性具有较好的辅助诊断价值。
【Objective】To explore the diagnostic value of 256iCT thin-slice scanning combined with three-dimensional reconstruction technology for solitary pulmonary nodule(SPN).【Methods】From December 2016 to June 2021,167 patients with CT manifestations of SPN in the outpatients and inpatient of our hospital were selected as the research objects.All were pathological confirmation,and were divided into benign group(60 cases)and malignant group(107 cases).The general data such as the patient’s age were collected,and the patient’s nodule type,nodule size,lobular sign,burr sign,tumor-lung interface,vascular cluster sign and so on were detected by 256iCT thin-layer scan.The consistency of 256iCT thin-slice scanning combined with three-dimensional reconstruction technology in the judgment of SPN benign and malignant and pathological examination results was analyzed by Kappa.The diagnostic value of 256iCT thin-slice scanning combined with three-dimensional reconstruction technology for SPN benign and malignant was analyzed.【Results】Compared with the benign group,the age,nodule type,nodule size,lobular sign,burr sign,tumor-lung interface,vascular cluster sign and other factors were significantly different in the malignant group(P<0.05).The diagnosis results of 256iCT thin-slice scan showed a high consistency with the pathological diagnosis(Kappa=0.685,P<0.05),and the combined detection diagnosis results showed a high consistency with the pathological diagnosis(Kappa=0.802,P<0.05).The sensitivity of 256iCT thin-slice scanning combined with three-dimensional reconstruction technology was higher than that of 256iCT thin-slice scanning alone(P<0.05).【Conclusion】256iCT thin-slice scanning combined with three-dimensional reconstruction technology is a non-invasive technique for detecting the nature of SPN,and it has good auxiliary diagnostic value for judging benign and malignant SPN.
