摘要
目的探讨CT肾动脉成像在肾癌患者术前评估中的应用价值,为泌尿外科肾癌切除术提供影像学技术支持。方法分析118例肾癌患者CTA资料,应用三维重建技术重建肾动脉图像,观察肾动脉解剖、变异;肿瘤的形态特征及肿瘤的肾内、外供血动脉情况。结果肿瘤位于左肾者66例、右肾者52例,肿瘤最大径(4.92±1.68)cm;肾动脉变异率为40.68%(48/118),其中副肾动脉27例,肾动脉提早分支14例,混合型7例;变异发生于患侧26例(22.03%,26/118),健侧22例(18.64%,22/118),差异无统计学意义(P>0.05);肿瘤侧肾动脉分支及集合系统呈各种受压、受侵改变,瘤旁肾动脉分支增粗,肿瘤血管聚集;发现3例副肾动脉及5例提早分支为明确的肿瘤供血动脉;发现肿瘤肾外迷走供血动脉11支,主要来自脾动脉、肠系膜下动脉、肝动脉及膈下动脉。结论 256层CTA可准确评估肾癌患者的肾动脉解剖、变异及肿瘤供血动脉情况,对肾脏肿瘤切除术具有重要的临床指导价值。
Objective To explore the application value of CT renal artery imaging in preoperative evaluation of renal cell carcinoma, and to provide imaging technology support for renal cell carcinoma resection in Department of urology. Methods The CTA data of 118 cases of renal carcinoma were analyzed. The images of renal artery were reconstructed by three-dimensional reconstruction technique. The anatomy and variation of renal artery, the morphological characteristics of the tumor and the condition of the intrarenal and external blood supply of the tumor were observed. Results The tumor was located in 66 cases of left kidney, 52 cases of right kidney, the maximum diameter of tumor was (4.92±1.68) cm, renal artery variation rate was 40.68%(48/118), including 27 cases of accessory renal artery, 14 early renal artery branches, 7 mixed type, 26(22.03%, 26/118) cases in the affected side, 22(18.64%, 22/118) cases in the contralateral side, and the difference was not statistically significant ( P >0.05);the branches of the renal artery and the collection system of the tumor side were subjected to various compression and invasion. The branches of the renal artery were thickened and the tumor vessels gathered;we found 3 cases of accessory renal artery and 5 cases of early branch blood supply arteries clear;and 11 branches of the blood supply artery were found out of the kidney, mainly from the splenic artery, the inferior mesenteric artery, the hepatic artery, and the inferior phrenic artery. Conclusion The 256-layer CTA can accurately evaluate the renal artery anatomy, variation and the blood supply artery of the renal cancer patients. It has important clinical guiding value for the resection of renal tumor.
