细胞DNA定量分析技术在肺癌诊断中的应用价值 被引量:5

摘要 目的探讨细胞DNA定量分析技术在肺癌诊断中的应用价值。方法选取2019年9月至10月于首都医科大学附属北京胸科医院病理科检测的102例标本,包括痰样本48例、胸腹水样本31例和支气管灌洗液标本23例,每例标本制成2张液基薄层细胞片,分别行HE染色和Feulgen染色,由细胞学医师对HE染色片做常规细胞学诊断,应用全自动DNA倍体分析仪对Feulgen染色片进行自动扫描。以病理组织学结果作为金标准,评估细胞DNA定量分析技术和液基细胞学检查在肺癌诊断中的价值。结果(1)细胞DNA定量分析技术诊断肺癌的敏感度和特异度分别为73.4%和69.6%,液基细胞学检查诊断肺癌的敏感度和特异度分别为54.4%和82.6%,细胞DNA定量分析技术敏感度高于液基细胞学检查,差异有统计学意义(P<0.05)。2种方法联合检测的敏感度和特异度分别为77.2%和69.6%。(2)在不同来源的标本中比较,细胞DNA定量分析技术在支气管灌洗液中的敏感度高于痰标本(P<0.05)。(3)在不同病理组织分型的肺癌标本中,细胞DNA定量分析技术在腺癌中的敏感度高于小细胞癌(P<0.05)。结论细胞DNA定量分析技术在肺癌诊断中有很好的应用价值,与液基细胞学检查联合应用,二者优势互补,有利于肺癌的筛查和早期诊断。 Objective To explore the value of DNA image cytometry(DNA-ICM)in lung cancer diagnosis.Methods A total of 102 specimens were obtained from Department of Pathology,Beijing Chest Hospital,Capital Medical University from September to October 2019,including 48 cases of sputum,31 cases of thoracic-peritoneal effusion,and 23 cases of bronchial lavage fluid.Two slides were prepared from each case:one was stained with hematoxylin and eosin for conventional liquid-based cytology(LBC)examination,and the other was stained by Feulgen method for determination of DNA amount using automated DNA-ICM method.Diagnostic value of DNA-ICM and LBC in lung cancer was evaluated using the results of pathological biopsy as the golden standard.Results(1)The sensitivity and specificity of DNA-ICM in diagnosing lung cancer were 73.4%and 69.6%,and those of LBC were 54.4%and 82.6%,respectively.The sensitivity of DNA-ICM was significantly higher than that of LBC(P<0.05).Combining the above two methods,the sensitivity and specificity were 77.2%and 69.6%,respectively.(2)As for the specimen type,the sensitivity of DNA-ICM in alveolar lavage fluid was higher that in sputum(P<0.05).(3)The sensitivity of DNA-ICM in adenocarcinoma was higher than that in small cell carcinoma among different histopathological classifications of lung cancer(P<0.05).Conclusion DNA-ICM is valuable in diagnosing lung cancer.Combining of LBC and DNA-ICM can further improve the detection rate of tumors.This combined method is promising for early screening and diagnosing of lung cancer.
