摘要
目的探讨针刺联合温脾汤对老年慢性功能性便秘患者炎症因子、胃肠激素及生活质量的影响。方法选择2018年3月—2020年4月治疗的110例老年慢性功能性便秘患者为研究对象,按照计算机随机数字表法分为对照组(55例)和观察组(55例),对照组给予温脾汤治疗,观察组给予针刺联合温脾汤治疗。4周后比较2组患者的临床疗效和临床症状、血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)和γ干扰素(IFN-γ)等炎症因子水平,胃动素(MTL)、血管活性肠肽(VIP)和生长抑素(SS)等胃肠激素水平,生活质量量表(QLS)评价生活质量。结果观察组患者的总有效率为94.55%显著高于对照组的72.73%(P<0.05)。观察组患者的大便艰涩、拘急冷痛、口淡不渴和四肢不温评分显著低于对照组(P<0.05)。观察组血清TNF-α、IL-6和IFN-γ水平显著低于对照组(P<0.05)。观察组患者血清MTL水平高于对照组,而VIP和SS水平显著低于对照组(P<0.05)。观察组患者的身体、角色、心理、社会和认知功能评分显著高于对照组(P<0.05)。结论针刺联合温脾汤对老年慢性功能性便秘患者具有较好的临床疗效,可减轻炎症因子水平,改善胃肠激素,提高生活质量。
Objective To explore the effect of acupuncture combined with Wenpi decoctionon inflammatory factors,gastrointestinal hormones and quality of life in elderly patients with chronic functional constipation.Methods 110 elderly patients with chronic functional constipation who were treated in the outpatient department of our hospital from March 2018 to April 2020 were selected as the research objects.According to the computer random number table,they were divided into the control group(55 cases)and the observation group(55 cases).The control group was given Wenpi decoction,while the observation group was given acupuncture combined with Wenpi decoction.After 4 weeks,the clinical efficacy and clinical symptoms,serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interferon-γ(IFN-γ)levels,and gastric motility were compared between the two groups.The levels of gastrointestinal hormones such as MTL,vasoactive intestinal peptide(VIP)and somatostatin(SS),and the quality of life scale(QLS)were used to evaluate the quality of life.Results The total effective rate of patients in the observation group was 94.55%,which was significantly higher than 72.73%of the control group(P<0.05).The difficult stool,cold pain,not thirsty and hypothermia of limb score of the observation group were significantly lower than those of the control group(P<0.05).Serum TNF-α,IL-6 and IFN-γlevels in the observation group were significantly lower than those in the control group(P<0.05).The serum MTL level of the observation group was higher than which of the control group,while the VIP and SS levels of the observation group were significantly lower than the control group(P<0.05).The physical,role,psychological,social and cognitive function scores of the observation group were significantly higher than those of the control group(P<0.05).Conclusion Acupuncture combined with Wenpi decoction have good clinical effect on elderly patients with chronic functional constipation,it can reduce the level of inflammatory factors,and improve gastrointestinal hormones and the quality of life.
