摘要
目的观察针刺联合毫火针治疗心脾两虚型失眠的临床疗效及对患者焦虑、抑郁状态的影响。方法将75例心脾两虚型失眠患者按照随机数字表法分为2组,治疗组39例予针刺联合毫火针治疗,对照组36例予单纯针刺治疗。2组均6 d为1个疗程,疗程间休息1 d,共治疗2个疗程,疗程结束后2周随访。比较2组疗效;比较2组治疗前后及随访时匹兹堡睡眠质量指数(PSQ I)评分、失眠严重指数量表(ISI)评分、疲劳严重程度(FSS)评分、贝克焦虑量表(BAI)评分、贝克抑郁量表(BDI)评分变化;评价2组安全性。结果治疗组总有效率88.9%(32/36),对照组总有效率76.5%(26/34),治疗组疗效优于对照组(P<0.05)。2组治疗后及随访时PSQI睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物、日间功能障碍评分及总评分均较本组治疗前降低(P<0.05);治疗组治疗后及随访时PSQI入睡时间、睡眠时间、睡眠效率、睡眠障碍、日间功能障碍评分及总评分均低于对照组同期(P<0.05)。2组治疗后、随访时ISI、FSS、BAI、BDI评分均较本组治疗前降低(P<0.05);2组随访时ISI、BDI评分均较本组治疗后升高(P<0.05);治疗后、随访时治疗组ISI、FSS、BAI、BDI评分均低于对照组同期(P<0.05)。结论针刺联合毫火针治疗心脾两虚型失眠疗效确切,可改善患者疲劳、焦虑、抑郁程度。
Objective To investigate the clinical efficacy of acupuncture combined with filiform-fire needle on insomnia of heart-spleen deficiency.Methods Totally 75 patients with insomnia of heart-spleen deficiency were randomly divided into two groups,39 cases in treatment group were treated with acupuncture combined with filiform-fire needle,36 cases in control group were given acupuncture only.Six days for a course,patients were treated for two courses except one day rest,and follow-up was carried out 2 weeks after treatment.Clinical curative effect,Pittsburgh Sleep Quality Index(PSQI),Insomnia Severity Index(ISI),Fatigue Severity Scale(FSS),Baker Anxiety Inventory(BAI),as well as Baker Depression Inventory(BDI)between groups were compared before treatment and after treatment and follow-up(2 weeks after treatment),and the safety was evaluated.Results The total effective rates in treatment group were better than those in control group[88.9%(32/36)vs 76.5%(26/34),(P<0.05),respectively].Compared with before treatment,scores of sleep quality,time before falling asleep,sleep time,sleep efficiency,sleep disorders,hypnotic drugs,and Daytime dysfunction in both groups were decreased after treatment and during follow-up(P<0.05),which in treatment group were significantly decreased(P<0.05);scores of ISI,FSS,BAI and BDI in both groups were decreased after treatment and during follow-up(P<0.05),which in treatment group were significantly decreased(P<0.05).Compared with after treatment,scores of ISI and BDI in both groups were increased during follow-up(P<0.05).Conclusion For patients with insomnia of heart-spleen deficiency,acupuncture combined with filiform-fire needle has a definite effect,can improve fatigue,anxiety and depression of patients.
