摘要
目的:比较1.5T核磁共振成像(MRT)与多层螺旋CT(MSCT)对肩峰下撞击综合征的诊断价值。方法:2019年1月-2020年1月收治肩峰下撞击综合征患者54例,分别利用1.5T MRI和MSCT检查。比较两种检查方式的诊断灵敏度、约登指数、诊断特异性、Bigliani检出率等指标。结果:MRI诊断的灵敏度与约登指数均高于MSCT,差异有统计学意义(P<0.05);MRI与MSCT诊断特异性比较,差异无统计学意义(P>0.05)。在MRI与MSCT检查中,其肩峰段形态BiglianiⅠ型与Ⅱ型的检出率相比,差异无统计学意义(P>0.05);MRI在肩峰段形态BiglianiⅢ型的检出率高于MSCT,差异有统计学意义(P<0.05)。结论:在肩峰下撞击综合征诊断中,MRI与MSCT的特异性虽然无差异,但灵敏度前者高于后者,其对软组织的分辨率更高,无辐射,临床推广可能性更强。
Objective:To compare the diagnostic value of 1.5T MRI and multi-slice spiral CT(MSCT)for subacromial impingement syndrome.Methods:From January 2019 to January 2020,54 patients with subacromial impingement syndrome were seleccted.1.5T magnetic resonance imaging(MRI)and MSCT were used.We compared the indicators of diagnostic sensitivity,Youden index,diagnostic specificity,Bigliani detection rate,etc under the two inspection methods.Results:The sensitivity diagnosis and the Youden index of MRI were higher than MSCT,the difference was statistically significant(P<0.05).The specificity of MRI and MSCT was not statistically significant(P>0.05).In MRI and MSCT examination,the detection rate of the acromion morphology Bigliani typeⅠandⅡwas not statistically different(P>0.05).The detection rate of BiglianiⅢacromion morphology of MRI was higher than that of MSCT,the difference was statistically significant(P<0.05).Conclusion:In the diagnosis of subacromial impingement syndrome,although the specificity of 1.5T MRI and multi-slice spiral CT is not different,the sensitivity of the former is higher than that of the latter,and its resolution to soft tissues is higher without radiation.The potential for clinical replication is much stronger.
