摘要
目的:比较磁共振成像(MRI)与超声内镜在食管癌术前T分期诊断中的应用价值。方法:回顾性收集2018年1月至2020年12月于商丘市第一人民医院完成手术治疗的30例食管癌患者的临床资料,纳入患者均于术前接受MRI与超声内镜检查,且在检查后2周内接受手术治疗。以病理分期为"金标准",分析术前MRI、超声内镜检查对食管癌T分期的诊断价值。观察两种检查方式的主要影像学特征。结果:MRI诊断食管癌T分期与病理分期的符合率为93.33%(28/30),超声内镜诊断食管癌T分期与病理分期符合率为80.00%(24/30),两种诊断方式符合率比较差异未见统计学意义(P>0.05)。以病理分期为"金标准",MRI诊断食管癌T分期与病理分期的一致性(Kappa=0.906)优于超声内镜(Kappa=0.716)。MRI中以DWI高信号显示食管癌的T1~T4期,而超声内镜以高低回声相间的5层结构显示食管癌的T1~T4期。结论:术前MRI诊断食管癌T分期与病理结果有较高的符合率,诊断结果与病理结果的一致性优于超声内镜。
Objective To compare the value of magnetic resonance imaging(MRI)and endoscopic ultrasonography(EUS)in the diagnosis of preoperative T stage of esophageal cancer.Methods The clinical data of 30 patients with esophageal cancer who underwent surgical treatment in Shangqiu First People’s Hospital from January 2018 to December 2020 were retrospectively collected.All patients underwent MRI and EUS examination before operation,and received surgical treatment within 2 weeks after examination.The pathological stage was taken as"gold standard"to analyze the value of preoperative MRI and EUS examination in diagnosis of T stage of esophageal cancer.And the main imaging features of the two examination methods were observed.Results In the diagnosis of T stage of esophageal cancer,the coincidence rate of MRI to pathological staging was 93.33%(28/30),and the coincidence rate of EUS to pathological staging was 80.00%(24/30),but there was no significant difference in coincidence rate of the two diagnostic methods(P>0.05).Taking the pathological stage as"gold standard",the consistency of MRI(Kappa=0.906)was superior to that of EUS(Kappa=0.716)in diagnosing T stage of esophageal cancer.High signal intensity of DWI suggested T1 to T4 stage of esophageal carcinoma in MRI;the 5-layer structure with high and low echo suggested T1 to T4 stage of esophageal carcinoma in EUS.Conclusions Preoperative MRI diagnosis has a high coincidence rate to pathological results in diagnosing T stage of esophageal cancer,and the consistency of MRI to pathological results is superior to that of EUS.
