摘要
目的评价康柏西普与雷珠单抗治疗视网膜中央静脉阻塞(CRVO)继发黄斑水肿的效果。方法回顾性研究。汕头国际眼科中心2016年1月至2019年6月收治CRVO继发黄斑水肿共40例(40只眼),患者分为视盘水肿组25例与非视盘水肿组15例,每个月1次玻璃体内注射抗VEGF药物(康柏西普或雷珠单抗),连续3个月,分析视盘水肿对治疗的影响。结果视盘水肿组3次治疗后视力为0.73±0.48,优于治疗前的1.17±0.58(t=4.28,P=0.001)。非视盘水肿组3次治疗后视力为0.66±0.27,优于治疗前的1.12±0.48(t=3.91,P=0.002)。两组间治疗前与治疗后视力差异均无统计学意义(P>0.05)。多元回归分析发现基线视力、椭圆体带完整性与预后视力相关。结论使用康柏西普或雷珠单抗治疗CRVO继发黄斑水肿,视盘水肿组与非视盘水肿组最终视力均有增进,但两组间无明显差异。
Objective To evaluate the efficacy of conbercept and ranibizumab in the treatment of macular edema secondary to central retinal vein occlusion(CRVO).Methods This was a retrospective study.A total of 40 eyes from 40 patients with macular edema secondary to CRVO from January 2016 to June 2019 at the Joint Shantou International Eye Center(JSIEC)were included in this study.These patients were divided into the optic disc edema group(25 cases)and non-optic disc edema group(15 cases).All patients were treated with intravitreal injection of conbercept or ranibizumab once a month for 3 consecutive months,and the effect of optic disc edema were analyzed.Results The visual acuity of patients in the optic disc edema group and non-optic disc edema group was 0.73±0.48 and 0.66±0.27,respectively after three times of treatment.Both were better than pre-treatment visual acuity(1.170.58,t=4.28,P=0.001;1.12±0.48,t=3.91,P=0.002),but the before and after treatment difference was not statistically significant between the two groups(P>0.05).Multiple regression analysis revealed that baseline vision and ellipsoidal band integrity were associated with visual acuity prognosis.Conclusions The visual acuity of patients with macular edema secondary to CRVO improved after intravitreal injection of conbercept or ranibizumab,but the extent of improvement was not statistically different between the optic disc edema group and non-optic disc edema group.
