摘要
目的:观察人工晶状体巩膜层间固定术在眼球挫伤所致晶状体脱位中应用的临床效果。方法:回顾性分析汕头大学香港中文大学部分合汕头国际眼科中心2018年1月至2019年12月眼球挫伤致晶状体全脱位或不全脱位者58例(58只眼)行晶状体摘出联合人工晶状体巩膜层间固定术的临床资料。术后随访(91.5±61.4)d,观察术后视力、眼压、屈光状态及手术并发症。结果:本研究晶状体脱位范围超过3个象限者30例(51.72%),需行后段玻璃体切除术,其中需重水辅助者8例(13.79%),合并高眼压28例(48.28%)。术后视力(logMAR)为0.76±0.56,优于术前的1.52±0.57(t=9.71,P<0.05)。术后眼压为(14.3±5.4)mmHg,低于术前的(27.5±16.4)mmHg(t=5.95,P<0.05)(1 mmHg=0.133 kPa)。术中1例人工晶状体坠入玻璃体内。术后3例眼压高于21 mmHg,1例出现前房积血。无脉络膜爆发性出血、眼内炎或视网膜脱离等严重并发症发生。术后人工晶状体位置均居中,无明显偏移。结论:人工晶状体巩膜层间固术可用于眼挫伤所致晶状体脱位患者一期治疗,操作简单,安全有效。
Objective To observe the clinical efficacy of intrascleral fixation of intraocular lens in luxation of lens caused by ocular contusion.Methods The clinical data of 58 eyes of 58 cases with subluxation or complete luxation of lens due to ocular contusion treated with lens removal and intrascleral fixation of intraocular lens from Jan.2018 to Dec.2019 in Shatou University Chinese University of Hongkong Joint Shantou International Eye Center were analyzed retrospectively.The follow-up time was 3 months.The postoperative visual acuity,intraocular pressure,refractive status and surgery complication were observed.Results Thirty cases(51.72%)underwent posterior vitrectomy due to that the range of lens dislocation exceeded 3 quadrants,among which 8 cases(13.79%)required liquid perfluorocarbon assistance,and 28 cases(48.28%)with high intraocular pressure.The visual acuity(logMAR)improved from 1.52±0.57 before surgery to 0.76±0.56 after surgery(t=9.71,P<0.05).The intraocular pressure decreased from(27.5±16.4)mmHg before surgery to(14.3±5.4)mmHg after surgery(t=5.95,P<0.05)(1 mmHg=0.133 kPa).One case of intraocular lens fell into the vitreous cavity during the operation.The intraocular pressure of 3 cases were higher than 21 mmHg and 1 case with hyphema after surgery.There was no serious complication such as explosive choroidal hemorrhage,endophthalmitis or retinal detachment.The position of intraocular lens was centered after operation.Conclusion Intrascleral fixation of intraocular lens can be used for the first-stage treatment of patients with luxation of lens caused by ocular contusion.The procedure of operation is simple,safe and effective.
