摘要
目的探讨左卡尼汀联合维生素E在弱精子症患者辅助生殖中的应用价值.方法选择2019年5月—2020年5月上饶市妇幼保健院收治的弱精子症患者80例,按随机数字表法分为2组,各40例.对照组采用维生素E治疗,研究组加用左卡尼汀,对比2组精子质量、临床妊娠率、优质胚胎率及不良反应.结果治疗前,2组前向运动精子(PR)、前后精子活动力(PR+NP)、精子DNA碎片率、精子畸形率相比,差异无统计学意义(P>0.05);治疗后,研究组PR(33.49±7.10)%、PR+NP(49.56±9.32)%,高于对照组的(27.12±6.48)%、(37.41±7.85)%,精子DNA碎片率(21.26±1.14)%、精子畸形率(35.21±1.03)%,低于对照组的(24.15±1.37)%、(39.38±1.27)%,差异有统计学意义(P<0.05);研究组临床妊娠率、优质胚胎率分别为65.00%,37.50%,高于对照组的42.50%,15.00%,差异有统计学意义(P<0.05);2组不良反应发生率差异无统计学意义(P>0.05).结论左卡尼汀联合维生素E能够改善弱精子症患者精子质量,增加临床妊娠率、优质胚胎率,且安全可靠.
Objective To investigate the value of L-carnitine combined with vitamin E in assisted reproduction of asthenozoospermic patients.Methods Eighty patients with asthenozoospermia admitted to our hospital from May 2019 to May2020 were selected and divided into two groups according to random number table,with 40 cases in each group.The control group was treated with vitamin E and the study group was treated with L-carnitine.The sperm quality,clinical pregnancy rate and adverse reactions of high quality embryos were compared between the two groups.Results Before tr eatment,there was no significant difference in progressive motile sperm(PR),anterior and posterior sperm motility(PR+NP),sperm DNA fragmentation rate and sperm malformation rate between the two groups(P>0.05);after treatment,PR(33.49±7.10)%and PR+NP(49.56±9.32)%in study group were higher than(27.12±6.48)%and(37.41±7.85)%in control group,sperm DNA fragmentation rate(21.26±1.14)%and sperm malformation rate(35.21±1.03)%in study group were lower than(24.15±1.37)%and(39.38±1.27)%in control group,and the difference had statistical significance(P<0.05);clinical pregnancy rate and high-quality embryo rate in study group were 65.00%and 37.50%,respectively,higher than 42.50%and 15.00%in control group,and the difference had statistical significance(P<0.05);the incidence rate of adverse reactions was similar between the two groups,and the difference had no statistical significance(P>0.05).Conclusion L-carnitine combined with vitamin E can improve the sperm quality of patients with asthenozoospermia,increase the clinical pregnancy rate and high-quality embryo rate,and is safe and reliable.
