血清SAA、hs-CRP、Cys-C和尿微量白蛋白联合检测在糖尿病肾病早期诊断的临床应用 被引量:17

Clinical application of combined detection of serum SAA,hs CRP,Cys-C and microalbuminuria in early diagnosis of diabetic nephropathy

摘要 目的探讨血清SAA、hs-CRP、Cys-C和尿微量白蛋白联合检测在糖尿病肾病早期诊断中的临床应用价值。方法选取2020年6月至2021年5月本院收治的200例糖尿病患者,按尿微量白蛋白排泄率分为单纯糖尿病组(n=57)、早期糖尿病肾病组(n=53)、中期糖尿病肾病组(n=45)和晚期糖尿病肾病组(n=45),同时,选取同期在本院进行健康体检者100名作为对照组,比较各组血清CysC、SAA、hs-CRP及尿mALB检测结果,各项血清指标检测阳性率及四项联合血清指标检测阳性率。结果对照组SAA、CysC、hs-CRP及尿mALB水平低于单纯糖尿病组,单纯糖尿病组低于早期糖尿病肾病组,早期糖尿病肾病组低于中期糖尿病肾病组,中期糖尿病肾病组低于晚期糖尿病肾病组;四项联合指标检测阳性率为93.00%,高于单项血清指标(P<0.05)。结论血清SAA、CysC、hs-CRP及尿mALB指标水平均可作为糖尿病肾病的生物学指标,四者联合检测可有效提升糖尿病肾病早期诊断准确性,值得临床推广应用。 Objective To investigate the clinical value of combined detection of serum SAA,hs CRP,Cys-C and microalbuminuria in the early diagnosis of diabetic nephropathy.Methods 200 cases of diabetic patients in our hospital from June 2020 to May 2021 were selected and they were divided into simple diabetes group(n=57),early diabetic nephropathy group(n=53),medium-term diabetic nephropathy group(n=45),and late diabetic nephropathy group(n=45)according to the urinary microalbuminuria excretion rate.Meanwhile,100 healthy volunteers in our hospital during the same period were selected as the control group,the results of serum Cysc,SAA,hs-CRP and urinary MALB in each group,and the positive rates of various serum indicator detection and combined detection of four serum indicators were compared.Results The levels of SAA,CysC,hs-CRP and urine mALB in the control group were lower than those in the simple diabetes group,while those in the simple diabetes group were lower than those in the early diabetic nephropathy group,while those in the early diabetic nephropathy group were lower than those in the middle stage diabetic nephropathy group,while those in the middle stage diabetic nephropathy group were lower than those in the late diabetic nephropathy group;the positive rate of combined detection of four serum indicators was 93.00%,which was higher than each single serum index detection(P<0.05).Conclusion Serum SAA,CysC,hs-CRP and urine mAlb can be used as biological indicators of diabetic nephropathy.The combined detec-tion of the four indicators can effectively improve accuracy of the early diagnostic of diabetic nephropathy,which is worthy of clinical promotion and application.
