摘要
目的:分析血清胱抑素C(CysC)、25-羟基维生素D[25-(OH)D]水平与2型糖尿病(T2DM)患者肾损伤发生的相关性.方法:选取本院2018年8月至2020年8月收治的98例T2DM患者,入院当天检测患者血清CysC、25-(OH)D水平,次日检测尿微量白蛋白(ALB)、肌酐(Cr)水平并计算ALB/Cr比值(ACR),评估患者肾损伤发生情况.将患者分为肾损伤发生组和肾损伤未发生组,分析CysC、25-(OH)D水平与T2DM患者肾损伤的相关性.结果:纳入的98例T2DM患者,24例发生肾损伤,初步比较两组一般资料后,经Logistics回归分析显示,血清CysC水平高(OR=11.655,95%CI:1.370-99.175),25-(OH)D水平低(OR=1.573,95%CI:1.276-1.939)与T2DM患者肾损伤的发生有关(P<0.05).绘制受试者工作特征曲线图(ROC)显示,血清CysC、25-(OH)D单独及联合预测T2DM患者肾损伤曲线下面积(AUC)分别为0.785、0.815、0.853,均有一定预测价值.结论:血清CysC水平高、25-(OH)D水平低与T2DM患者发生肾损伤有关,血清CysC、25-(OH)D联合检测对T2DM患者发生肾损伤有较高预测价值.
Objective:To determine the relationship between serum cystatin C(CysC),25 hydroxyvitamin D(25-(OH)D)levels and renal injury in type 2 diabetes mellitus patients(T2DM).Methods:98 patients with T2DM treated in our hospital between August 2018 and August 2020 were selected;serum CysC and 25-(OH)D levels were determined on the day of admission;urinary microalbumin(ALB)and creatinine(CR)levels were determined and the ALB/Cr ratio(ACR)was calculated the following day;the occurrence of renal injury in patients was evaluated and divided into occurrence and non-occurrence groups;the correlation between CysC,25-(OH)D levels and renal injury in T2DM patients was analyzed.Results:Of the 98 patients with T2DM included in the study,24 developed kidney injury.After comparing the general data of the two groups,logistic regression analysis revealed that a high serum CysC level(OR=11.655,95%confidence interval[CI]:1.370-99.175)and a low 25-(OH)D level(OR=1.573,95%CI:1.276–1.939)were associated with renal injury in T2DM patients(P<0.05).The receiver operating characteristic curve(ROC)analysis revealed that the area under the renal injury curve(AUC)of serum CysC and 25-(OH)D for single and combined were 0.785,0.815,and 0.853 prediction of renal injury in T2DM patients respectively,indicating that they had some predictive value.Conclusion:While both elevated serum CysC and low 25-(OH)D levels are associated with renal injury in T2DM patients,detecting serum CysC and 25-(OH)D together has a higher predictive value for renal injury in T2DM patients.
