同型半胱氨酸与尿微量白蛋白联合检测在糖尿病肾病中的临床诊断价值分析 被引量:3

Analysis of Clinical Diagnostic Value of Combined Detection of Homocysteine and Urinary Microalbumin in Diabetic Nephropathy

摘要 目的分析同型半胱氨酸(homocysteine,Hcy)和尿微量白蛋白(urine microalbumin,U-mALB)联合检测在糖尿病肾病中的临床诊断价值。方法于2020年8月—2021年7月选取70例糖尿病肾病患者及70名同期健康体检者纳入研究,均采取Hcy及U-mALB检测,分析这两项指标在糖尿病肾病诊断中的应用价值。结果糖尿病肾病患者血清Hcy、U-mALB检测结果均高于健康体检者,差异有统计学意义(P<0.05),且糖尿病肾病中晚期患者的Hcy、U-mALB水平明显高于早期患者,差异有统计学意义(P<0.05)。Hcy联合U-mALB诊断糖尿病肾病,诊断灵敏度为90.0%(63/70),特异度为97.1%(68/70),准确性为93.6%(131/140)。结论糖尿病肾病患者血清Hcy及U-mALB指标有明显改变,通过这两项指标的综合分析,可提升糖尿病肾病的诊断灵敏度及特异度。 Objective To analyze the clinical diagnostic value of combined detection of homocysteine(Hcy)and urine microalbumin(U-mALB)in diabetic nephropathy.Methods From August 2020 to July 2021,and 70 patients with diabetic nephropathy and 70 healthy subjects during the same period were selected for inclusion in the study.Hcy and U-mALB were used for detection.The application value of these two indicators in the diagnosis of diabetic nephropathy was analyzed.Results The detection results of serum Hcy and U-mALB in patients with diabetic nephropathy were higher than those in healthy people,and the difference was statistically significant(P<0.05).And the levels of Hcy and U-mALB in patients with advanced diabetic nephropathy were significantly higher than those in early stage patients,and the difference was statistically significant(P<0.05).Hcy combined with U-mALB to diagnose diabetic nephropathy,the diagnostic sensitivity was 90.0%(63/70),the specificity was 97.1%(68/70),and the accuracy was 93.6%(131/140).Conclusion The serum Hcy and U-mALB indexes in patients with diabetic nephropathy have significant changes.The comprehensive analysis of these two indexes can improve the diagnostic sensitivity and specificity of diabetic nephropathy.
