摘要
目的:观察四妙清瘟败毒饮加减治疗儿童EB病毒相关传染性单核细胞增多症(infectious mononucleosis,IM)的临床疗效。方法:将符合纳入标准的70例IM患儿按照随机数字表法分为对照组和治疗组,每组各35例。对照组给予更昔洛韦注射液,治疗组患儿在对照组基础上联合四妙清瘟败毒饮治疗。比较两组患儿的临床疗效、临床症状缓解时间及治疗前后中医证候积分变化情况,检测两组患儿治疗后获得性免疫应答反应相关指标(CD4~+、CD8~+、CD4~+/CD8~+)、肝功能[丙氨酸氨基转移酶(alanine transaminase,ALT)、天冬氨酸氨基转移酶(aspartateaminotransferase,AST)]、异常淋巴细胞比率及EBDNA变化情况。结果:治疗组有效率为94.28%,对照组有效率为80.00%,两组患儿有效率比较,差异具有统计学意义(P<0.05)。治疗组患儿发热、咽峡炎、颈部淋巴结、肝脾肿大好转等时间短于对照组,差异具有统计学意义(P<0.05)。两组患儿治疗后中医证候积分低于本组治疗前,治疗后治疗组低于对照组,差异具有统计学意义(P<0.05)。治疗后治疗组CD4~+、CD4~+/CD8~+、ALT、AST高于对照组,CD8~+、异常淋巴细胞比率、EBDNA低于对照组,差异具有统计学意义(P<0.05)。结论:四妙清瘟败毒饮加减治疗儿童EB病毒相关IM,可提高患儿获得性免疫应答反应,改善中医证候,缩短临床症状缓解时间。
Objective:To observe the clinical curative effect of Modified Wonderfully Effective Four Epidemic-Clearing Toxin-Resolving Beverage on epstein-barr virus related infectious mononucleosis(IM)in children.Methods:A total of 70 IM children who met the inclusion criteria were divided into the control group and the treatment group according to random number table,with 35 cases in each group.The control group was given Ganciclovir Injection,while the treatment group was treated with Modified Wonderfully Effective Four Epidemic-Clearing Toxin-Resolving Beverage on the basis of the control group.The clinical efficacy,remission time of clinical symptoms and changes of TCM syndrome score before and after treatment were compared between the two groups.The acquired immune response related indicators(CD4+,CD8+,CD4+/CD8+),liver functions[including alanine transaminase(ALT),and aspartate aminotransferase(AST)],abnormal lymphocyte ratio and changes in EBDNA were detected after treatment.Results:The effective rate was 94.28%in the treatment group and 80.00%in the control group.There was a significant difference between the two groups(P<0.05).The fever,angina,cervical lymph nodes,hepatosplenomegaly and other improvements in the treatment group were shorter than those in the control group,and all the differences were statistically significant(P<0.05).After treatment,the score of TCM syndromes in the two groups was lower than that in the control group,and the score of TCM syndromes of the treatment group was lower than that of the control group,and the difference was statistically significant(P<0.05).After treatment,CD4+,CD4+/CD8+,ALT,AST in the treatment group were higher than those in the control group,and CD8+,abnormal lymphocyte ratio,and EBDNA were lower than those in the control group,with statistical significance(P<0.05).Conclusion:In terms of epstein-barr virus related infectious mononucleosis(IM)in children,Modified Wonderfully Effective Four Epidemic-Clearing Toxin-Resolving Beverage can improve the acquired immune response of children,enhance TCM syndromes and shorten the improvement time of clinical symptoms.
