摘要
目的探讨系统性红斑狼疮患者几种实验室指标在早期肾损伤中的应用价值及肾损伤危险因素。方法以2016年1月至2019年12月入住青岛市市立医院,初诊系统性红斑狼疮患者为研究对象,探讨各相关指标在系统性红斑狼疮肾损伤患者中的改变。结果 IgG、IgA、Cys-C、β_(2)-MG、UA、RDW水平在系统性红斑狼疮患者中明显高于对照组,而RBC、HB、C3、C4、A/G则明显低于对照组。尿蛋白阳性的系统性红斑狼疮患者IgG、IgA、C3、A/G、RBC、HB水平显著低于尿蛋白阴性患者,而Cys-C、β_(2)-MG、UA、RDW水平则显著升高(P<0.01)。β_(2)-MG、Cys-C、UA水平在肾功能正常组、肾功能异常组、肾功能损害组依次上升,各组表达水平均存在显著性差异;RBC、IgG水平在肾功能损害组较其他两组显著下降,其HB、C3、A/G水平较肾功能正常组明显降低。Logistic回归分析结果显示IgG、C3、UA、A/G水平是系统性红斑狼疮患者肾损伤独立危险因素。结论血清IgG、C3、A/G水平下降,UA水平升高是SLE患者肾损伤独立危险因素,联合检测Cys-C、β_(2)-MG及以上指标对诊断患者肾脏早期损害、病情进展提供可靠实验室依据。
Objective To explore the application value of several laboratory indicators in early renal injury of patients with systemic lupus erythematosus and the risk factors for renal injury.Methods Patients with newly diagnosed systemic lupus erythematosus who were admitted to the hospital from January 2016 to December 2019 were used as the research object to explore the changes of various related indicators in kidney injury of patients with systemic lupus erythematosus.Results The levels of IgG,IgA,Cys-C,β_(2)-MG,UA,RDW in patients with systemic lupus erythematosus were significantly higher than those in the healthy control group,while the levels of RBC,HB,C3,C4,A/G were significantly lower than those in the control group.The levels of IgG,IgA,C3,A/G,RBC,and HB in patients of systemic lupus erythematosus whose urine protein was positive were significantly lower than those with urine protein-negative patients,while the levels of Cys-C,β_(2)-MG,UA,RDW were significant increased(P<0.01).The levels ofβ_(2)-MG,Cys-C,and UA increased ascending in the group of normal renal function,abnormal renal function,the renal function impairment,and the expression levels of each group were significantly different.The levels of RBC and IgG in the renal function impairment group were higher than the other two groups.Compared with the normal renal function group,the levels of HB,C3 and A/G in the renal function impairment group were significantly decreased.Logistic binary regression analysis results showed that IgG,C3,UA,A/G was independent risk factors for renal injury in SLE patients.Conclusion Decreased serum IgG and C3 levels and increased serum UA levels are independent risk factors for renal injury in SLE patients.Combined detection of Cys-C,β_(2)-MG and above indicators provides reliable laboratory evidence for the diagnosis of early renal damage and disease progression in patients.
