摘要
目的探讨血清白细胞介素-2受体α(IL-2Rα)、25-羟维生素D(25(OH)D)及miR-200水平与系统性红斑狼疮(SLE)患者疾病活动度及预后的关系。方法选取2019年8月-2021年8月汉中市人民医院收治的102例SLE患者,根据SLE疾病活动度评分2000(SLEDAI-2K)将患者分为轻度活动组(<9分,n=36)、中度活动组(10~14分,n=30)和重度活动组(≥15分,n=36)。比较各组血清IL-2Rα、25(OH)D及miR-200水平差异;采用Pearson相关系数分析SLE患者血清IL-2Rα、25(OH)D及miR-200水平与疾病活动度的相关性。所有患者均随访1年,以复发或死亡为终点事件,分为预后良好组(n=74)和预后不良组(n=28),用logistic回归分析影响SLE患者预后不良的独立危险因素。结果重度活动组血清IL-2Rα水平高于中度活动组、轻度活动组,25(OH)D、miR-200水平低于中度活动组、轻度活动组,差异均有统计学意义(P均<0.05);且中度活动组血清IL-2Rα水平高于轻度活动组,25(OH)D、miR-200水平低于轻度活动组,差异均有统计学意义(P均<0.05)。Pearson相关性分析显示,血清IL-2Rα水平与SLE疾病活动度成正相关(r=0.495,P<0.001),血清25(OH)D、miR-200水平均与SLE疾病活动度成负相关(r=-0.355、-0.495,P均<0.001)。SLEDAI-2K评分、血肌酐(SCR)、血清IL-2Rα水平是SLE患者预后不良的独立危险因素(OR=1.543、1.814、1.563,P均<0.05),血清25(OH)D及miR-200水平是保护因素(OR=0.932、0.943,P均<0.05)。结论血清IL-2Rα表达升高及25(OH)D、miR-200表达减少与SLE疾病活动度有关,且能影响患者预后。
Objective To investigate the relationship of serum interleukin-2 receptorα(IL-2Rα),25-hydroxy vitamin D[25(OH)D]and miR-200 levels with disease activity and prognosis in patients with systemic lupus erythematosus(SLE).Methods A total of 102 SLE patients admitted to Hanzhong People's Hospital from August 2019 to August 2021 were selected.According to the SLE disease activity index 2000(SLEDAI-2K),the patients were divided into mild activity group(<9 points)36 cases,moderate activity group(10-14 points)30 cases and severe activity group(≥15 points)36 cases.The levels of serum IL-2Rα,25(OH)D and miR-200 in each group were compared.Pearson correlation coefficient was used to analyze the correlation between serum IL-2Rα,25(OH)D and miR-200 levels and disease activity in SLE patients.All patients were followed up for 1 year.With recurrence or death as the endpoint event,they were divided into good prognosis group(n=74)and poor prognosis group(n=28).Logistic regression analysis was used to analyze the independent risk factors affecting the poor prognosis of SLE patients.Results Serum IL-2Rαlevel in severe activity group was higher than that in moderate activity group and mild activity group,while 25(OH)D and miR-200 levels were lower than those in moderate activity group and mild activity group,the differences were statistically significant(P all<0.05).Serum IL-2Rαlevel in moderate activity group was higher than that in mild activity group,while 25(OH)D and miR-200 levels were lower than those in mild activity group,the differences were statistically significant(P all<0.05).Pearson correlation analysis showed that serum IL-2Rαlevel was positively correlated with SLE disease activity(r=0.495,P<0.001),while serum 25(OH)D and miR-200 levels were negatively correlated with SLE disease activity(r=-0.355,-0.495,P all<0.001).SLEDAI-2K score,serum creatinine(SCR)and serum IL-2Rαlevels were independent risk factors for poor prognosis in SLE patients(OR=1.543,1.814,1.563;P all<0.05).Serum 25(OH)D and miR-200 levels were protective factors(OR=0.932,0.943;P all<0.05).Conclusion Increased expression of serum IL-2Rαand decreased expression of 25(OH)D and miR-200 levels were associated with SLE disease activity and could affect the prognosis of patients.
