Characteristics of hand, foot and mouth disease identified in Beijing You′an Hospital from 2015 to 2019

摘要 目的分析2015-2019年北京佑安医院手足口病的病原谱及流行病学特征。方法采用描述性流行病学方法分析2015-2019年在北京佑安医院就诊的手足口病病例,结合测定的基因序列对部分非肠道病毒A组71型(enterovirus A71, EV-A71)和非柯萨奇病毒A组16型(coxsackievirus A, CV-A16)的其他肠道病毒进行基因分型。结果 2015-2019年北京佑安医院就诊的手足口病病例累计3 894例,EV-A71、CV-A16和其他肠道病毒病例的构成比分别为16.3%(636/3 894)、15.3%(594/3 894)和68.4%(2 664/3 894)。病例数自2017年显著降低,其他肠道病毒所占比例逐渐增高,2018-2019年EV-A71所占比例显著降低,2019年CV-A16所占比例回升。0~6岁的手足口病病例占总病例数的89.3%(3 479/3 894);非EV-A71和非CV-A16的其他肠道病毒感染患者的年龄偏小(H=33.327,P<0.001);病例中男女性别比例为1.56∶1;成年人感染占5.2%(201/3 894)。其他肠道病毒感染病例发病高峰后移是2017年后手足口病发病高峰从6月后移至7-8月的主要原因。2018-2019年成功鉴定的42例其他肠道病毒标本中,CV-A6所占比例最高,达64.3%(27/42),还有CV-A10、CV-A16、CV-A2和CV-A5等多种病原共流行。结论北京佑安医院就诊的手足口病病例数自2017年显著且持续降低,5年间非EV-A71和非CV-A16的其他肠道病毒是主要病原并显现出新的流行特征,应持续关注并加强监测。 Objective To analyze the pathogen spectrum and epidemiological characteristics of hand, foot and mouth disease(HMFD) identified in Beijing You′an Hospital from 2015 to 2019. Methods Descriptive epidemiological methods were used for data analysis of HFMD cases recruited from Beijing You′an Hospital during 2015-2019, and sequencing was performed to identify other enterovirus of non-enterovirus A71(EV-A71) and non-coxsackievirus A16(CV-A16) in specimens obtained from 2018 to 2019. Results Altogether there were 3 894 cases positive for enterovirus of HFMD in Beijing You′an Hospital during 2015 and 2019.The percentages of EV-A71, CV-A16 and other enterovirus were 16.3%(636/3 894), 15.3%(594/3 894) and 68.4%(2 664/3 894), respectively.From 2017 the number of HFMD cases significantly decreased, while the cases caused by other enteroviruses increased.In 2018 and 2019, HFMD related with EV-A71 decreased significantly, whereas cases with CV-A16 was high in 2019.Patients aged 0-6 years accounted for 89.3%(3 479/3 894), whereas infections caused by other enteroviruses occurred at younger ages(H=33.327, P<0.001).The ratio of males to females was 1.56 to 1, and 5.2%(201/3 894) of total cases were adults.The late onset of other enterovirus infections appeared to be the reason for the incidence peak of overall cases that moved from June to July and August after 2017.During 2018 and 2019, 42 specimens positive for other enteroviruses were further identified,with the highest proportion of CV-A6,reaching 64.3%(27/42),as well as a co-prevalenceof other enteroviruses such as CV-A16,CV-A10,CV-A2 and CV-A5. Conclusions The number of HFMD cases identified in Beijing You′an Hospital has significantly decreased since 2017.However,other enteroviruses of non-EV-A71 and non-CV-A16 become prevalent with new characteristics during the study period.
