摘要
目的了解本地区3年内女性泌尿生殖系统支原体感染分布情况以及耐药谱的变化,为本地区支原体感染患者诊治提供可靠依据。方法采取简单随机抽样法,抽取2017年1月~2019年12月1522例患者,并对支原体培养结果和体外药敏结果进行回顾性分析。结果1522例标本中611例支原体培养结果呈阳性,检出率为40.14%。其中解脲脲原体(Uu)感染、人型支原体(Mh)感染及两者混合感染的检出率分别为30.03%、1.64%和8.48%。阴道炎、宫颈炎、盆腔炎、尿道炎以及混合泌尿生殖道感染的患者支原体检出率高于健康体检者(P<0.05)。支原体培养阳性的患者年龄集中分布在21~40岁,且夏季的检出率(47.22%)高于其他季节(P<0.05)。Uu和Mh单独感染和联合感染时,对交沙霉素、美满霉素和强力霉素的敏感性较高;对环丙沙星、氧氟沙星、左氧氟沙星普遍耐药。结论本地区女性支原体培养阳性标本中主要以Uu为主,其检出率与年龄、季节、泌尿生殖道感染有一定相关性。交沙霉素、美满霉素和强力霉素可作为本地区治疗支原体感染的首选药物,结合培养与药敏结果用药更加合理。
Objective To understand the distribution of mycoplasma infection in female urogenital tract and the change of drug resistance spectrum in the past three years,and to provide reliable basis for diagnosis and treatment of mycoplasma infection in this area.Methods From January 2017 to December 2019,a total of 1522 patients were selected by simple random sampling,and the results of mycoplasma culture and drug sensitivity in vitro were analyzed retrospectively.Results Among 1522 specimens,611 cases were positive for mycoplasma culture,and the detection rate was 40.14%.The detection rates of Ureaplasma urealyticum(Uu)infection,mycoplasma hominis(Mh)infection and their mixed infection were 30.03%,1.64%and 8.48%,respectively.The detection rates of mycoplasma in patients with vaginitis,cervicitis,pelvic inflammatory,urethritis and mixed urogenital tract infection were higher than those in healthy people(P<0.05).The ages of mycoplasma positive patients ranged from 21 to 40 years old,and the detection rate in summer(47.22%)was higher than that in other seasons(P<0.05).Uu and Mh were highly sensitive to josamycin,minocycline hydrochloride and doxycycline when infected alone or in combination.Meanwhile,they were generally resistance to ciprofloxacin,ofloxacin and levofloxacin.Conclusion Uu is the main positive specimen of female mycoplasma culture in this area,and its detection rate is related to age,season and urogenital tract infection.Josamycin,minocycline hydrochloride and doxycycline are used as the first choice for the treatment of mycoplasma infection in this area,and they are more reasonable to be used in combination with the results of culture and drug sensitivity.
