300例HPV阳性患者支原体和衣原体及淋球菌感染情况及药敏性分析 被引量:6

Investigation on infection and drug sensitivity of mycoplasma,chlamydia and gonococcus in 300 HPV positive patients

摘要 目的探讨人乳头瘤病毒(HPV)阳性患者支原体、沙眼衣原体和淋球菌感染情况及药敏性分析。方法选取2021年1-12月在承德医学院附属医院确诊的300例HPV阳性女性患者为研究对象,采集所有患者宫颈分泌物或阴道分泌物进行支原体、衣原体和淋球菌的检查,并对阳性样本进行药敏性分析。结果300例HPV阳性患者中,占比前5的HPV亚型分别为52型、16型、58型、18型和51型;合并生殖道感染阳性检出率为80.00%;其中支原体感染、沙眼衣原体、淋球菌感染阳性检出率分别为74.00%、11.67%、3.00%,支原体感染率较高(χ^(2)=132.346,P<0.05)。四重及以上HPV亚型患者支原体和衣原体感染率较单一HPV亚型患者高,差异有统计学意义(χ^(2)=3.956、7.316,P<0.05);三重HPV亚型患者淋球菌感染率较单一HPV亚型患者高,差异有统计学意义(χ^(2)=5.326,P=0.021)。支原体对强力霉素、交沙霉素、克拉霉素、阿奇霉素敏感,对环丙沙星、美满霉素较耐药;衣原体对罗红霉素、阿奇霉素、强力霉素敏感,对环丙沙星较耐药;淋球菌对阿奇霉素、大观霉素、头孢克肟敏感,对环丙沙星、青霉素耐药。结论HPV阳性患者均有感染支原体、衣原体和淋球菌的风险,其中支原体感染风险较高,且三重及以上HPV阳性患者较单一患者生殖道感染风险高;三类病原体均对环丙沙星耐药。 Objective To explore the infection and drug sensitivity of mycoplasma,Chlamydia trachomatis and gonococcus in human papillomavirus(HPV)positive patients.Methods A total of 300 female HPV positive patients confirmed in the Affiliated Hospital of Chengde Medical University were enrolled as the research objects between January and December2021.The cervical or vaginal secretions were collected to detect mycoplasma,chlamydia and gonococcus.Drug susceptibility analysis was performed on positive samples.Results Among the 300 HPV positive patients,HPV subtypes with proportions on the top five were types 52,16,58,18 and 51.The positive detection rate of reproductive tract infection was 80.00%.The positive detection rates of mycoplasma,Chlamydia trachomatis and gonococcus infection were 74.00%,11.67%and 3.00%,respectively.The incidence of mycoplasma infection was the highest(χ^(2)=132.346,P<0.05).The infection rates of mycoplasma and chlamydia in patients with quadruple or more HPV subtypes were higher than those with single subtype(χ^(2)=3.956,7.316,P<0.05).The infection rate of gonococcus in patients with triple HPV subtype was higher than that with single subtype(χ^(2)=5.326,P=0.021).Mycoplasma was sensitive to doxycycline,clarithromycin,azithromycin and josamycin,and resistant to ciprofloxacin and minocycline.Chlamydia was sensitive to roxithromycin,azithromycin and doxycycline,and resistant to ciprofloxacin.Gonococcus was sensitive to azithromycin,spectinomycin and cefixime,and resistant to ciprofloxacin and penicillin.Conclusions There was risk of mycoplasma,chlamydia and gonococcus infection in HPV-positive patients.The risk of mycoplasma infection was the highest.The risk of reproductive tract infection in patients with triple or more HPV subtype was higher than that with single subtype.All the three kinds of pathogens were resistant to ciprofloxacin.
