摘要
目的分析布地奈德联合肺泡表面活性物质气管内滴入治疗对早产新生儿呼吸窘迫综合征(RDS)的临床效果。方法选取2015年2月至2019年2月解放军联勤保障部队第九八八医院开封院区收治的早产儿RSD患者72例为研究对象,依据随机双盲原则分为对照组与观察组,每组36例。对照组予以肺泡表面活性物质进行气管内滴入治疗,观察组在对照组的基础上加用布地奈德治疗。比较两组患儿的临床效果及呼吸机使用时间和撤机时间以及住院时间,同时比较两组患儿治疗前后血气指标。结果治疗后,观察组治疗总有效率(94.44%,34/36)高于对照组(69.44%,25/36),差异有统计学意义(χ2=7.604,P<0.05)。观察组患儿呼吸机使用时间、撤机时间以及住院时间均明显短于对照组,差异有统计学意义(P<0.05)。结论早产RSD患儿采用布地奈德联合肺泡表面活性物质气管内滴入治疗可有效提高临床效果,改善血气指标,且有助于缩短患儿的治疗时间,效果确切,值得临床推广。
Objective To analyze the clinical effect of budesonide combined with pulmonary surfactant intratracheal instillation on premature neonates with respiratory distress syndrome(RDS).Methods From February 2015 to February 2019,72 premature infants with RSD admitted to Kaifeng Hospital of the 98 th Hospital of the joint service support force of the PLA were selected as the study objects.According to the principle of double blindness,the patients were divided into control group and observation group,with 36 cases in each group.The control group was treated by intratracheal instillation of pulmonary surfactant,and the observation group was treated with budesonide on the basis of the control group.The clinical effects,ventilator use time,weaning time and hospitalization time of the two groups were compared.At the same time,the blood gas indexes of the two groups before and after treatment were compared.Results After treatment,the total effective rate of the observation group(94.44%,34/36)was higher than that of the control group(69.44%,25/36),the difference was significant(χ2=7.604,P<0.05).The ventilator use time,weaning time and hospitalization time in the observation group were significantly shorter than those in the control group(P<0.05).Conclusions The combination of budesonide and pulmonary surfactant can effectively improve the clinical effect of premature RSD,which can improve the blood gas index and shorten the treatment time of children.The effect is accurate and worthy of clinical promotion.
