摘要
目的:探讨不同体质量指数多囊卵巢综合征(PCOS)患者的临床及内分泌特征。方法:招募肥胖型和非肥胖型PCOS患者各20例和健康女性15名。观察并比较两种体质量指数患者的人体测量学指标、血清性激素水平、空腹血糖、胰岛素释放试验。结果:①肥胖组多毛、痤疮评分均低于非肥胖组(P<0.05,P<0.01),而黑棘皮症发生率在这二组中无显著差异。②生化指标:肥胖组的黄体生成素(LH)、黄体生成素/卵泡刺激素(LH/FSH)、雌二醇(E2)水平均低于非肥胖组(P<0.01,P<0.05)。胰岛素释放试验肥胖组0h、2h胰岛素水平均高于非肥胖组(P<0.01,P<0.05)。非肥胖组的胰岛素敏感指数(ISI)高于肥胖组(P<0.01),而胰岛素曲线下面积(IAUC)、空腹血糖(FBG)低于肥胖组(P<0.05)。结论:肥胖型PCOS患者较非肥胖PCOS患者存在更严重的内分泌代谢紊乱。
Objective:To explore the clinical and endocrine characteristics of patients with different body mass index polycystic ovary syndrome(PCOS).Methods:Forty PCOS patients(20 of obese and 20 of non-obese)and 15 healthy women were recruited.The anthropometric indexes,serum sex hormone levels,fasting blood glucose and insulin release test of patients with two kinds of BMI were observed and compared.Results:①The scores of hirsutism and acne in the obese group were lower than those in the non-obese group(P<0.05,P<0.01),there was no significant difference in the incidence of acanthosis nigricans between the two groups.②Biochemical indexes:The levels of luteinizing hormone(LH),luteinizing hormone/follicle stimulating hormone(LH/FSH),and estradiol(E2)in the obese group were lower than those in the non-obese group(P<0.01,P<0.05).The 0 h,2 h insulin levels in the obese group were higher than those in the non-obese group(P<0.01,P<0.05).The insulin sensitivity index(ISI)was higher in the non-obese group than in the obese group(P<0.01),while insulin area under the curve(IAUC)and fasting blood glucose(FBG)were lower in the obese group(P<0.05).Conclusion:Obese PCOS patients have more severe endocrine metabolic disorders than non-obese PCOS patients.
